摘要

目的:通过超声检测胎儿脐动脉(UA)和大脑中动脉(MCA)血流参数,评价其在预测胎儿宫内窘迫的价值.方法:选取 2021 年 6 月至 2022 年 9 月在云浮市妇幼保健院检查的晚孕期孕妇 100 例为研究对象,把临床诊断为胎儿宫内窘迫的 50 例孕妇设为观察组,把临床产检胎儿无异常的 50 例孕妇设为对照组.对所有胎儿的UA和MCA血流参数进行规范测量,记录阻力指数(RI)、搏动指数(PI)、收缩期峰值流速与舒张末期流速比值(S/D)并进行组间比较.结果:观察组胎儿UA的RI、PI、S/D均高于对照组,MCA的RI、PI均低于对照组,差异均具有统计学意义(P<0.05);观察组胎儿的MCA和UA的RI比值、MCA和UA的PI比值均较对照组更低,差异均具有统计学意义(P<0.05).结论:检测胎儿UA和MCA血流阻力指标参数可以反映胎儿宫内缺氧情况,能够预测胎儿宫内窘迫的发生.

Abstract

Objective To evaluate the value of fetal umbilical artery(UA)and middle cerebral artery(MCA)blood flow parameters in predicting fetal distress by ultrasonography.Methods A total of 100 pregnant women in late pregnancy who were examined in Maternal and Child Health Hospital of Yunfu from June 2021 to September 2022 were selected as the study objects,50 pregnant women with clinical diagnosis of fetal distress were set as the observation group,and 50 pregnant women with no abnormal fetuses were set as the control group.The UA and MCA blood flow parameters of all fetuses were measured,and the resistance index(RI),pulsatility index(PI),the ratio of peak systolic velocity/end diastolic velocity(S/D)were recorded and compared between groups.Results The RI,PI and S/D of fetal UA in the observation group were higher than those in the control group,RI and PI of MCA were lower than those of the control group,and the differences were statistically significant(P<0.05).The RI ratio and PI ratio of MCA and UA in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Detecting fetal UA and MCA blood flow resistance indexes can reflect fetal hypoxia in utero and predict the occurrence of fetal distress.
