目的:探讨转化生长因子β(TGFβ)、基质金属蛋白酶-2(MMP-2)在青光眼术后滤过泡瘢痕化中的意义及影响因素.方法:选取漯河医学高等专科学校第二附属医院 2021 年 6 月至 2023 年 6 月收治的 80 例青光眼患者作为研究对象,所有患者均采取青光眼小梁切除术治疗,对所有患者进行 4 个月随访,将术后发生滤过泡瘢痕化的 23 例患者纳入滤过泡瘢痕化组,将术后未发生滤过泡瘢痕化的 57 例患者纳入非滤过泡瘢痕化组,对比两组患者TGFβ、MMP-2 水平,采用Pearson检验分析TGFβ、MMP-2 与青光眼术后滤过泡瘢痕化发生的相关性.并对比两组患者临床一般资料,采用logistic回归模型分析发生滤过泡瘢痕化的影响因素.结果:滤过泡瘢痕化组患者血清TGFβ、MMP-2水平明显高于非滤过泡瘢痕化组,差异具有统计学意义(P<0.05).Pearson分析结果显示TGFβ、MMP-2水平与青光眼术后滤过泡瘢痕化发生呈正相关(r=0.49,P<0.05;r=0.54,P<0.05).两组患者术前眼压、晶体厚度、术后并发滤过泡渗漏情况比较,差异具有统计学意义(P<0.05);滤过泡渗漏、TGFβ、MMP-2 是术后滤过泡瘢痕化发生的影响因素(P<0.05).结论:TGFβ、MMP-2 水平与青光眼术后滤过泡瘢痕化发生具有明显相关性,且术后并发滤过泡渗漏、TGFβ、MMP-2 水平可影响青光眼术后滤过泡瘢痕化发生.
Objective To investigate the significance and influencing factors of transforming growth factor β(TGFβ)and matrix metalloproteinase-2(MMP-2)in postoperative glaucoma filtration vesicle scarring.Methods Eighty glaucoma patients admitted to the Second Affiliated Hospital of Luohe Medical College from June 2021 to June 2023 were selected as the study subjects,all patients were treated with glaucomatous trabeculectomy,all patients were followed up for 4 months,23 patients with postoperative filtration vesicle scarring were included in the filtration vesicle scarring group,and 57 patients who did not have postoperative filtration vesicle scarring were included in the non filtered vesicle scarring group,the levels of TGFβ and MMP-2 of patients were compared in the two groups,and the correlation between TGFβ and MMP-2 and the occurrence of postoperative filtered vesicle scarring in glaucoma was analyzed by Pearson test.The clinical general data of the two groups of patients were compared,and the influencing factors of the occurrence of filtered vesicle scarring were analyzed by logistic regression model.Results The serum levels of TGFβ and MMP-2 in the filtered vesicle scarring group were significantly higher than those in the non-filtered vesicle scarring group,and the differences were statistically significant(P<0.05).Pearson analysis results showed that the levels of TGFβ and MMP-2 were positively correlated with the occurrence of postoperative filtered vesicle scarring in glaucoma(r=0.49,P<0.05;r=0.54,P<0.05).The comparison of preoperative intraocular pressure,crystalline lens thickness,and postoperative complications of filtration vesicle leakage between the two groups showed statistically significant differences(P<0.05),and the filtration vesicle leakage,TGFβ,and MMP-2 were the influencing factors for the occurrence of postoperative filtration vesicle scarring(P<0.05).Conclusion TGFβ and MMP-2 levels were significantly correlated with the occurrence of postoperative filtration vesicle scarring in glaucoma,and postoperative concurrent filtration vesicle leakage,TGFβ and MMP-2 levels could influence the occurrence of postoperative filtration vesicle scarring in glaucoma.
