中药益气活血法结合耳穴埋豆治疗化疗药物致外周神经炎的临床研究

摘要
目的:分析中药益气活血法结合耳穴埋豆在化疗药物致外周神经炎患者中的临床应用效果.方法:选择泰和县中医院2022 年 1 月至2022 年 12 月期间使用长春碱类、铂类、紫杉类等药物治疗的患者70 例,按照随机数字表法将其分为对照组与观察组,各35 例.对照组患者采取单纯西药治疗,观察组患者在对照组的基础上采用中医辨证施治,使用中药益气活血法结合耳穴埋豆治疗,两组患者均进行为期2周的治疗.比较两组患者外周神经炎发生率、外周神经炎症状持续时间与严重程度、外周神经炎并发症改善程度、临床疗效.结果:观察组患者外周神经炎发生率为52.43%,低于对照组的77.14%,差异具有统计学意义(P＜0.05);观察组患者外周神经炎持续时间为(17.62±2.85)d,短于对照组的(27.98±3.11)d,差异具有统计学意义(P＜0.05);观察组患者严重程度轻于对照组,差异具有统计学意义(Z=19.707,P＜0.05);观察组患者外周神经并发症评分(肢体麻木、肌力减退、跟腱反射减退)均低于对照组,差异具有统计学意义(P＜0.05);观察组患者治疗总有效率为94.44%,高于对照组的62.96%,差异具有统计学意义(P＜0.05).结论:中药益气活血法结合耳穴埋豆在化疗药物致外周神经炎患者中具有良好的治疗效果,能显著缓解患者临床症状,缩短症状持续时间,改善神经病变程度,促进外周神经炎并发症的改善.
Abstract
Objective To analyze the clinical application effect of the traditional Chinese medicine Yiqi Huoxue method combined with auricular acupoint bean embedding in patients with chemotherapy-induced peripheral neuropathy.Methods A total of 70 patients treated with vinca alkaloids,platinum,and taxanes at Taihe County Hospital of Traditional Chinese Medicine from January 2022 to December 2022 were selected and divided into a control group and an observation group according to the random number table method,with 35 cases in each group.Patients in the control group were treated with western medicine alone,while patients in the observation group were additionally treated with traditional Chinese medicine based on syndrome differentiation,using the Yiqi Huoxue method combined with auricular acupoint bean embedding.Both groups were treated for 2 weeks.The incidence of peripheral neuropathy,duration and severity of symptoms,improvement of complications,and clinical efficacy were compared between the two groups.Results The incidence of peripheral neuropathy in the observation group was 52.43%,which was lower than 77.14%in the control group,and the difference was statistically significant(P<0.05);the duration of peripheral neuropathy in the observation group was(17.62±2.85)days,which was shorter than(27.98±3.11)days in the control group,and the difference was statistically significant(P<0.05);the severity of symptoms in the observation group was less than that in the control group,and the difference was statistically significant(Z=19.707,P<0.05);the scores of peripheral neuropathy complications(limb numbness,muscle strength reduction,achilles tendon reflex reduction)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05);the total effective rate of treatment in the observation group was 94.44%,which was higher than 62.96%in the control group,and the difference was statistically significant(P<0.05).Conclusion The traditional Chinese medicine Yiqi Huoxue method combined with auricular acupoint bean embedding has a good therapeutic effect in patients with chemotherapy-induced peripheral neuropathy,significantly alleviating clinical symptoms,shortening the duration of symptoms,improving the degree of neuropathy,and promoting the improvement of complications of peripheral neuropathy.
