Application of Integrated Extended Care in Patients with Type 2 Diabetes Mellitus and Its Influence on Blood Glucose Control and Self-management Ability
Objective To explore the application of integrated extended care in patients with type 2 diabetes and its in-fluence on blood glucose control and self-management ability. Methods A total of 110 elderly patients with type 2 dia-betes diagnosed and treated in Xuzhou Hospital of Traditional Chinese Medicine from May 2022 to August 2023 were se-lected as the study objects, and were divided into control group(n=55,routine nursing intervention)and observation group(n=55,integrated extended care intervention)according to random number table method.Blood glucose levels, health behaviors,self-management ability and quality of life were compared between the two groups of patients. Results After nursing, the blood glucose index of observation group was lower than that of control group, and the differ-ence was statistically significant(P<0.05).The health behavior score of the observation group(185.85±16.39)points was higher than that of the control group(154.39±14.50)points,and the difference was statistically significant(t= 10.662,P<0.05).The self-management ability score of the observation group was higher than that of the control group, and the difference was statistically significant(P<0.05).The life quality score of the observation group was lower than that of the control group, and the difference was statistically significant(P<0.05).Conclusion The application of inte-grated extended care in patients with type 2 diabetes is beneficial to blood glucose control, improve health behavior, significantly improve their self-management ability and quality of life, and the effect is ideal.
