达格列净联合胰岛素泵在初诊2型糖尿病患者中的应用分析

摘要
目的 分析初诊2型糖尿病(Diabetes Mellitus Type 2,T2DM)患者采用达格列净联合胰岛素泵治疗取得的效果.方法 选取2022年7月—2023年6月山东国欣颐养集团枣庄医院治疗的96例T2DM初诊患者为研究对象,以随机数表法分为两组,每组48例.对照组接受胰岛素泵治疗,观察组在其基础上联用达格列净治疗.比较两组首次血糖达标时每日胰岛素用量、每日胰岛素用量(停泵前)、血糖达标时间、胰岛素抵抗指数、胰岛β细胞功能指数,以及不良反应发生情况.结果 观察组血糖首次达标时每日胰岛素用量(43.60±8.44)U、停泵前每日胰岛素用量(25.50±7.50)U均低于对照组的(52.65±7.58)U、(32.40±7.85)U,血糖达标时间(4.79±1.78)d短于对照组的(6.50±2.05)d,差异有统计学意义(t=5.527、4.403、4.364,P均<0.05).治疗后,观察组胰岛素抵抗指数低于对照组,胰岛β细胞功能指数高于对照组,差异有统计学意义(P均<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论 初诊T2DM患者在胰岛泵的基础上联用达格列净不仅能够促进血糖快速达标,减少胰岛素用量,且改善胰岛功能.
Abstract
Objective To analyze the effect of dapagliflozin combined with insulin pump in newly diagnosed patients with type 2 diabetes mellitus(T2DM).Methods A total of 96 newly diagnosed T2DM patients treated in Zaozhuang Hospital of Shandong Guoxin Yiyang Group from July 2022 to June 2023 were selected as the study objects and di-vided into two groups by random number table method,with 48 cases in each group.The control group was treated with insulin pump,and the observation group was treated with daglipzin.The daily insulin dosage by the time of first blood glucose compliance,daily insulin dosage(before pump withdrawal),blood glucose compliance time,insulin re-sistance index,islet beta cell function index,and the occurrence of adverse reactions were compared between the two groups.Results The daily insulin dosage by the time of first blood glucose compliance(43.60±8.44)U and the daily in-sulin dosage(25.50±7.50)U before pump stop in the observation group were lower than those of the control group(52.65±7.58)U and(32.40±7.85)U,the time for blood glucose to reach the standard(4.79±1.78)d was shorter than that of the control group(6.50±2.05)d,and the difference was statistically significant(t=5.527,4.403,4.364,all P<0.05).After treatment,the insulin resistance index of the observation group was lower than that of the control group,and the pancreatic islets β cell function index was higher than that of the control group,and the differences were sta-tistically significant(both P<0.05).There was no statistically significant difference in the incidence of adverse reac-tions between the two groups(P>0.05).Conclusion In newly diagnosed T2DM patients,the combination of insulin pump and dapagliflozin can not only promote rapid blood glucose control,reduce insulin dosage,but also improve pan-creatic function.
