尿葡萄糖检验结合尿微量白蛋白检验对糖尿病早期肾损伤的诊断分析

摘要
目的 分析尿葡萄糖(Glucose,GLU)+尿微量白蛋白(Urinary Microalbumin,UmALB)诊断糖尿病早期肾损伤(Early Renal Injury,ERI)的效果.方法 选取2020年9月—2022年11月厦门湖里国宇健康管理中心收治的60例糖尿病患者作为研究对象,依据ERI是否发生分为A组和B组,各30例,另外选取同期健康体检的30例健康人为对照组.对比3组尿GLU、UmALB结果,分析尿GLU+UmALB联合检验的诊断效能.结果 A组、B组尿GLU、UmALB高于对照组,B组尿GLU、UmAL高于A组,差异有统计学意义(P均<0.05).尿GLU+UmALB联合检验的敏感度为93.33%(28/30)、特异度为98.33%(59/60)、准确度为96.67%(87/90).尿GLU+UmALB联合检验与确诊结果具有一致性(Kappa值为0.92).结论 尿GLU+UmALB联检可明确ERI发生,敏感度、特异度等均高.
Abstract
Objective To analyze the effect of urinary glucose(GLU)and urinary microalbumin(UmALB)in the diag-nosis of early renal injury(ERI).Methods Sixty patients with diabetes admitted to Xiamen Huli Guoyu Health Man-agement Center from September 2020 to November 2022 were selected as the study objects.According to whether ERI occurred,they were divided into group A and Group B,with 30 cases each.In addition,30 healthy people who under-went physical examination during the same period were selected as the control group.The results of urinary GLU and UmALB were compared among the three groups,and the diagnostic efficacy of urinary GLU+UmALB combined test was analyzed.Results Urine GLU and UmALB in groups A and B were higher than those in control group,and urine GLU and UmAL in group B were higher than those in group A,the differences were statistically significant(all P<0.05).The sensitivity of urinary GLU+UmALB combined test was 93.33%(28/30),the specificity was 98.33%(59/60),the accuracy was 96.67%(87/90).Urine GLU+UmALB combined test was consistent with the confirmed results(the Kappa value was 0.92).Conclusion Urinary GLU+UmALB test can confirm the occurrence of ERI with high sensitiv-ity and specificity.
