摘要

目的 分析胰岛素不同给药方法治疗重症糖尿病酮症酸中毒的临床效果.方法 选取2023年1-10月泉州市第一医院重症医学科收治的88例重症糖尿病酮症酸中毒患者为研究对象,用随机数表法分为对照组和研究组,各44例.对照组予以小剂量胰岛素滴注治疗,研究组予以胰岛素泵持续治疗.对比两组治疗效果.结果 与对照组相比,研究组患者临床治疗总有效率更高、酸中毒纠正时间、尿酮体转阴时间更短、胰岛素使用量更少,差异有统计学意义(P均<0.05).治疗后,相比于对照组患者,研究组患者总抗氧化能力、超氧化物歧化酶水平更高,丙二醛水平更低,差异有统计学意义(P均<0.05).研究组患者低血糖发生率低于对照组,差异有统计学意义(P<0.05).结论 重症糖尿病酮症酸中毒患者选用胰岛素泵持续治疗可提高临床治疗效果,缩短酸中毒纠正时间以及尿酮体转阴时间,减少胰岛素使用量,还可改善机体氧化应激状态,降低低血糖发生风险.

Abstract

Objective To analyze the clinical effect of different insulin administration methods in the treatment of se-vere diabetic ketoacidosis.Methods A total of 88 patients with severe diabetic ketoacidosis admitted to Department of Critical Care Medicine,Quanzhou First Hospital from January 2023 to October 2023 were selected as the study ob-jects,and they were divided into the control group and the study group by random number table method.The control group was treated with low dose insulin drip,and the study group was treated with insulin pump continuously.The therapeutic effect of the two groups were compared.Results Compared with the control group,the total effective rate of clinical treatment in the study group was higher,the time of acidosis correction,the time of urine ketone body turn-ing negative were shorter,and the amount of insulin was less,the differences were statistically significant(all P<0.05).After treatment,compared with the control group,the total antioxidant capacity,superoxide dismutase level of the study group were higher,the level of malondialdehyde was lower,the differences were statistically significant(all P<0.05).The incidence of hypoglycemia in the study group was lower than that in the control group the difference was statistically significant(P<0.05).Conclusion Continuous treatment with insulin pump for patients with severe diabetic ketoacidosis can improve the clinical therapeutic effect,shorten the time of acidosis correction and the time of urine ketone body turning negative,reduce the amount of insulin,improve the oxidative stress state of the body,and reduce the risk of hypoglycemia.
