Effect of Panretinal Laser Photocoagulation Combined with Anti Vascular Endothelial Growth Factor Drugs in the Treatment of Diabetes Related Neovascular Glaucoma

目的 研究全视网膜激光光凝术联合抗血管内皮生长因子药物(Vescular Endothelial Growth Factor,VEGF)用于糖尿病相关的新生血管性青光眼患者临床治疗中的应用效果.方法 选取2018年9月—2022年9月福建医科大学附属南平市第一医院收治的70例糖尿病相关的新生血管性青光眼患者为研究对象,通过随机数表法分为对照组和观察组,各35例.对照组以全视网膜激光光凝术方案治疗,观察组则联合VEGF药物治疗.对比两组患者眼压水平、视网膜乳头周围神经纤维层(Peripapillary Retinal Never Fiber Layer,PNFL)厚度、视力水平、新生血管消失时间、并发症总发生率.结果 术前,两组患者眼压水平、PNFL厚度、视力水平比较,差异无统计学意义(P均>0.05);术后,观察组视力水平高于对照组,眼压水平低于对照组,PNFL厚度大于对照组,新生血管消退时间短于对照组,并发症总发生率低于对照组,差异有统计学意义(P均<0.05).结论 针对糖尿病相关新生血管性青光眼患者,在实施全视网膜激光光凝术治疗的过程中,通过应用VEGF药物可以获得显著的治疗效果.这一治疗方式有助于改善患者的症状,提高视力,并降低复发的风险.因此,对于糖尿病相关新生血管性青光眼患者,在全视网膜激光光凝术治疗的基础上联合应用VEGF药物是一种有效的治疗手段.
Objective To study the clinical effect of panretinal laser photocoagulation combined with anti vascular en-dothelial growth factor(VEGF)drugs in the treatment of diabetes related neovascular glaucoma.Methods A total of 70 patients with diabetic neovascular glaucoma admitted to the Nanping First Hospital,Affiliated to Fujian Medical University from September 2018 to September 2022 were selected as the study objects,and were divided into control group and observation group by random number table method,with 35 cases in each group.The control group was treated with whole-retina laser photocoagulation.The observation group was treated with VEGF drugs.The intraocular pressure level,peripapillary retinal never fiber layer(PNFL)thickness,visual acuity level,loss time of neovasculariza-tion and total incidence of complications were compared between the two groups.Results Before operation,there were no statistically significant difference in intraocular pressure,PNFL thickness and visual acuity between two groups(all P>0.05).After surgery,the visual acuity level of the observation group was higher than that of the control group,the in-traocular pressure level was lower than that of the control group,the PNFL thickness was greater than that of the con-trol group,the neovascularization time was shorter than that of the control group,and the total complication rate was lower,the difference were statistically significant(all P<0.05).Conclusion In the treatment of diabetes-related neovas-cular glaucoma,the application of VEGF drugs can achieve significant therapeutic effect in the process of full retinal laser photocoagulation.This treatment helps to improve the patient's symptoms,improve vision,and reduce the risk of recurrence.Therefore,for diabetes-related neovascular glaucoma patients,the combined application of VEGF drugs on the basis of total retinal laser photocoagulation treatment is an effective treatment.
