摘要
目的 探究尿液检查对糖尿病患者初期肾损伤诊断结果的影响.方法 选取2021年11月—2022年10月厦门大学附属第一医院收治的60例疑似早期肾损伤的糖尿病患者为研究对象,所有患者均检验尿液中尿微量白蛋白水平.以临床综合诊断结果(包括病史、肾功能检查、影像学检查等)为金标准,分析尿微量蛋白水平对肾损伤的诊断价值.结果 金标准结果可知,60例患者中45例阳性,15例阴性;尿微量白蛋白分别检出阳性41例、阴性19例,kappa值为0.826.以临床综合诊断结果为金标准,尿微量白蛋白诊断灵敏度、特异度、准确度分别为75.56%、53.33%、70.00%.结论 糖尿病患者尿液检查中尿微量白蛋白检验诊断早期肾损伤准确性更高,体现出较高价值.
Abstract
Objective To explore the influence of urine examination on the diagnosis of early renal injury in diabetic patients.Methods A total of 60 diabetic patients with suspected early renal injury admitted to the First Affiliated Hos-pital of Xiamen University from November 2021 to October 2022 were selected as the study objects.All patients were tested for urinary microalbumin levels.Using clinical comprehensive diagnosis results(including medical history,re-nal function examination,imaging examination,etc.)as the gold standard,the diagnostic value of urinary microprotein level in kidney injury was analyzed.Results The gold standard results showed that 45 of the 60 patients were positive and 15 were negative.Urinary microalbumin was detected positive in 41 cases and negative in 19 cases,and the kappa value was 0.826.The diagnostic sensitivity,specificity and accuracy of urinary microalbumin were 75.56%,53.33%and 70.00%.Conclusion Urine microalbumin examination in urine examination of diabetes patients is more accurate in the diagnosis of early renal injury,reflecting higher value.
