Application Effect of Triangle Stratified Hierarchical Management Com-bined with LEARNS Model in Nursing Care of Patients with Type 2 Diabe-tes Mellitus
Objective To investigate the application effect of Triangle stratified hierarchical management combined with LEARNS model in the care of Type 2 Diabetes Mellitus(T2DM)patients.Methods A total of 280 cases of T2DM patients admitted to Tianzhu Tibetan Autonomous County People's Hospital from February 2021 to February 2023 were selected as study objects and divided into conventional group(n=140,conventional care)and observation group(n=140,Triangle stratified hierarchical management combined with LEARNS mode on the basis of the conventional group)by the random envelope method.Observe and compare the differences between the groups before and after the care and between the indicators within the groups.Results Comparison of blood glucose level and self-management ability score between groups before intervention,and the differences were not significantly significant(both P>0.05).Comparison of blood glucose levels between groups after 6 months of intervention,the observation group was lower,and the difference was significantly significant(P<0.05).Comparison of self-management ability score after 6 months of intervention between groups,observation group(85.41±3.12)points was higher than the conventional group(84.23±3.19)points,and the difference was significantly significant(t=3.129,P<0.05).Conclusion Triangle stratified hierarchical management combined with LEARNS model of care for T2DM can improve blood glucose levels and pa-tients'self-management ability.
