胰岛素强化治疗的2型糖尿病患者低血糖的危险因素分析

摘要
目的 分析2型糖尿病(Type 2 Diabetes Mellitus,T2DM)患者接受胰岛素强化治疗时诱发其低血糖的危险因素.方法 回顾性选取2020年5月—2022年5月于武威市天祝藏族自治县人民医院接受胰岛素强化治疗的210例T2DM患者的临床资料,按照是否存在低血糖现象分为发生组(n=42)和未发生组(n=168).进行问卷调查(临床基线资料调查问卷等)及血液生化检测,对调查及检测结果展开单因素、Logistic多因素分析.结果 单因素分析结果显示,年龄、糖尿病病程、体质指数、C肽、肌酐、白蛋白均与胰岛素强化治疗的T2DM患者发生低血糖相关,差异有统计学意义(P均<0.05).Logistic回归分析结果显示,胰岛素强化治疗的T2DM患者低血糖发生的危险因素包括年龄(>60周岁)、体质指数(≤21 kg/m2)和糖尿病病程(>5年),差异有统计学意义(P均<0.05).结论 年龄、体质指数、糖尿病病程均为导致T2DM患者在胰岛素强化治疗中产生低血糖的危险因素.
Abstract
Objective To analyze the risk factor of hypoglycemia in Patients with type 2 diabetes mellitus(T2DM)treated with insulin intensive therapy.Methods Clinical data of two hundred and ten T2DM patients who received in-tensive insulin therapy in Tianzhu Tibetan Autonomous County People's Hospital of Wuwei City from May 2020 to May 2022 were retrospectively analyzed.According to whether there was hypoglycemia,they were divided into occur-rence group(n=42)and non-occurrence group(n=168).Questionnaire(clinical baseline data questionnaire)and blood biochemical test were selected to conduct univariate and Logistic multivariate analysis of the investigation and test re-sults.Results Analysis of the single factors of hypoglycemia in T2DM patients with insulin intensive treatment showed that age,diabetes course,body mass index,C-peptide,creatinine and albumin were correlated with hypoglycemia in T2DM patients with insulin intensive treatment,and the differences were statistically significant(all P<0.05).Logisti-cregression analysis showed that the risk factors for hypoglycemia in T2DM patients with insulin intensive treatment included age(>60 years old),body mass index(≤21 kg/m2),and duration of diabetes(>5 years),and the differences were statistically significant(all P<0.05).Conclusion Age,body mass index and duration of diabetes were risk factors for hypoglycemia in T2DM patients during intensive insulin therapy.
