HbA1c、FPG、2hPG及联合检验在糖尿病诊断中的效果及价值分析

摘要
目的 探讨生化检验在糖尿病患者中的效果及价值.方法 选取2021年6月—2023年8月寿光市人民医院收治的65例疑似糖尿病患者为研究对象,所有患者均采用全自动生化分析仪检测糖化血红蛋白(Glyco-sylated Hemoglobin A1c,HbA1c);采用全自动生化分析仪测定空腹血糖(Fasting Plasma Glucose,FPG)、餐后2 h血糖(2 Hour Postprandial Plasma Glucose,2 hPG),以临床最终诊断结果为金标准,分析生化检验指标在糖尿病患者中的检出率,并计算其诊断效能.结果 65例疑似糖尿病患者经金标准检查确诊46例;HbA1c检出阳性37例,FPG检出阳性35例,2 hPG检出阳性33例,联合检验(HbA1c+FPG+2 hPG)检出阳性43例;联合检验用于糖尿病诊断中准确度(83.08%)、灵敏度(84.78%)均高于单一指标检测,差异有统计学意义(χ2=6.294,7.129,P均<0.05).结论 HbA1c、FPG、2 hPG在糖尿病患者中具有较高检出率,联合检验能提升诊断价值.
Abstract
Objective To explore the effect and value of biochemical test in diabetic patients.Methods A total of 65 patients with suspected diabetes admitted to Shouguang People's Hospital from June 2021 to August 2023 were se-lected as the research objects.All patients were detected by automatic biochemical analyzer for glycosylated hemoglo-bin glycosylated hemoglobin A1c(HbA1c);fasting plasma glucose(FPG)and 2 hour postprandial plasma glucose(2 hPG)were measured by automatic biochemical analyzer.The detection rate of biochemical test indexes in diabetic pa-tients was analyzed with the final clinical diagnosis results as the gold standard,and the diagnostic efficiency was cal-culated.Results Among 65 patients with suspected diabetes,46 cases were diagnosed by gold standard examination.37 cases were positive for HbA1c,35 cases were positive for FPG,33 cases were positive for 2 hPG,and 43 cases were positive for combined detection(HbA1c+FPG+2 hPG).The accuracy(83.08%)and sensitivity(84.78%)of com-bined detection in the diagnosis of diabetes were higher than those of single index detection,and the differences were statistically significant(χ2=6.294,7.129,both P<0.05).Conclusion HbA1c,FPG and 2 hPG have a high detection rate in diabetic patients and joint testing can improve the diagnostic efficiency.
