低通量与高通量血液透析对2型糖尿病肾病患者应激反应及微炎症状态的影响研究

摘要
目的 探讨低通量与高通量血液透析在2型糖尿病肾病(Diabetic Nephropathy,DN)患者中的应用价值.方法 回顾性选取潍坊内分泌与代谢病医院于2021年5月—2023年6月收治的86例DN患者的临床资料,根据不同血液透析方式分为两组,各 43 例.对照组行低通量血液透析(Low-Flux Hemodialysis,LFHD),观察组行高通量血液透析(High Flux Hemodialysis,HFHD).对比两组肾功能指标、应激反应、微炎症状态.结果 治疗后,观察组血肌酐(380.21±20.18)μmol/L、血尿素氮(10.26±1.65)mmol/L、丙二醛(4.01±0.31)nmol/L、肿瘤坏死因子-α(72.09±5.25)ng/L、白介素-6(63.44±3.30)ng/L、超敏C反应蛋白(4.89±0.85)mg/L、降钙素原(1.53±0.20)μg/L低于对照组;肌酐清除率(37.89±3.55)mL/min、过氧化物歧化酶(89.01±2.44)U/mL、谷胱甘肽过氧化物酶(166.20±13.38)U/L高于对照组,差异有统计学意义(P均<0.05).结论 HFHD能有效改善DN患者肾功能、应激水平,减轻微炎症状态.
Abstract
Objective To investigate the application value of low-flux and high-flux hemodialysis in patients with type 2 diabetic nephropathy(DN).Methods Clinical data of eighty-six patients with DN admitted to Weifang Endo-crine and Metabolic Disease Hospital from May 2021 to June 2023 were retrospectively selected,and divided into 2 groups with forty-three cases each according to different hemodialysis methods.The control group had low-flux hemo-dialysis(LFHD),and the observation group had high-flux hemodialysis(HFHD).Renal function indexes,stress re-sponse and microinflammation were compared between the two groups.Results After treatment,the serum creatinine was(380.21±20.18)μmol/L,blood urea nitrogen was(10.26±1.65)mmol/L,malondialdehyde was(4.01±0.31)nmol/L,tumor necrosis factor-α was(72.09±5.25)ng/L and interleukin-6 was(63.44±3.30)ng/L,hypersensitive C-reactive protein was(4.89±0.85)mg/L,and procalcitonin was(1.53±0.20)μg/L in the observation group,lower than those of control group;creatinine clearance was(37.89±3.55)mL/min,peroxidase dismutase was(89.01±2.44)U/mL,glutathi-one peroxidase was(166.20±13.38)U/L in the observation group,higher than those of control group,and the differ-ences were statistically significant(all P<0.05).Conclusion HFHD can effectively improve renal function,stress level and reduce micro-inflammation in patients with DN.
