超声清创术+外用重组人粒细胞巨细胞刺激因子凝胶剂对糖尿病足Wagner Ⅱ、Ⅲ级溃疡治疗效果及愈合情况分析

摘要
目的 探究超声清创术+外用重组人粒细胞巨细胞刺激因子凝胶剂在糖尿病足Wagner Ⅱ、Ⅲ级溃疡治疗中的应用效果.方法 选取2023年2月—2024年2月江苏省靖江市人民医院内分泌科及烧伤整形科收治的70例糖尿病足Wagner Ⅱ、Ⅲ级溃疡患者为研究对象,以随机数表法分为参照组(35例,予以常规清创+胰岛素治疗)和研究组(35例,予以超声清创术+外用重组人粒细胞巨细胞刺激因子凝胶剂治疗).比较两组溃疡面愈合率、炎症因子水平及治疗总有效率.结果 治疗后2周,研究组溃疡创面愈合率略高于参照组,差异有统计学意义(P<0.05);治疗后1、2个月,研究组溃疡创面愈合率明显高于参照组,差异有统计学意义(P均<0.05).治疗前,两组降钙素原、超敏C反应蛋白水平比较,差异无统计学意义(P均>0.05).治疗后,两组降钙素原、超敏C反应蛋白水平均降低,且研究组显著低于参照组,差异有统计学意义(P均<0.05).研究组Wagner Ⅱ、Ⅲ级溃疡治疗总有效率高于参照组,差异有统计学意义(P<0.05).结论 超声清创术联合外用重组人粒细胞巨细胞刺激因子凝胶剂可促进糖尿病足Wagner Ⅱ、Ⅲ级溃疡面愈合,控制血清炎症因子水平,整体疗效显著.
Abstract
Objective To investigate the application effect of ultrasonic debridement+topical recombinant human granulocyte macrocyte stimulating factor gel in the treatment of diabetic foot Wagner grade Ⅱ and Ⅲ ulcers.Methods Seventy patients with diabetic foot Wagner grade Ⅱ and Ⅲ ulcers admitted to the Department of Endocrinology and Department of Burn and Plastic Surgery of Jingjiang People's Hospital from February 2023 to February 2024 were se-lected as the study objects,and they were divided into the reference group(35 cases,receiving conventional debride-ment+insulin treatment)and the study group(35 cases,receiving ultrasonic debridement+topical recombinant hu-man granulocyte macrocyte stimulating factor gel treatment)by random number table method.The healing rate of ulcer surface,the level of inflammatory factors and the total effective rate of two groups were compared.Results 2 weeks af-ter treatment,the ulcer wound healing rate of the study group was slightly higher than that of the reference group,and the difference was statistically significant(P<0.05).1 and 2 months after treatment,the ulcer wound healing rate of the study group were significantly higher than that of the reference group,and the differences were statistically signifi-cant(both P<0.05).Before treatment,there was no significant difference in the levels of procalcitonin and hypersensi-tive C-reactive protein between the two groups(both P>0.05).After treatment,the levels of procalcitonin and hyper-sensitive C-reactive protein were decreased in two groups,and the study group was significantly lower than the refer-ence group,and the differences were statistically significant(both P<0.05).The total effective rate of Wagner grade Ⅱand Ⅲ ulcer treatment in study group was higher than that in reference group,and the difference was statistically sig-nificant(P<0.05).Conclusion Ultrasonic debridement combined with topical recombinant human granulocyte macro-cyte stimulating factor gel can promote the healing of diabetic foot Wagner grade Ⅱ and Ⅲ ulcer surface,control the serum inflammatory factor level,and the overall efficacy of the treatment is remarkable.
