目的 探究胰岛素与恩格列净联合应用对老年2型糖尿病患者的抗氧化能力、血糖控制、胰岛素用量的影响.方法 回顾性选取2022年2月—2024年2月通用环球中铁咸阳医院收治的92例老年2型糖尿病患者的临床资料,依据不同治疗方法分为观察组(胰岛素联合恩格列净治疗)和对照组(胰岛素治疗),各46例.比较两组胰岛素抵抗指数(Homeostasis Model Assessment-IR,HOMA-IR)、血糖控制情况、胰岛素用量、抗氧化能力、空腹血糖达标时间.结果 治疗前,两组空腹血糖、HOMA-IR、餐后2 h血糖水平对比,差异无统计学意义(P均>0.05);治疗后,观察组空腹血糖、HOMA-IR、餐后2 h血糖水平均低于对照组,差异有统计学意义(P均<0.05).治疗前,两组谷胱甘肽过氧化物酶、丙二醛、活性氧以及血清超氧化物歧化酶水平对比,差异无统计学意义(P均>0.05);治疗后,观察组丙二醛、活性氧水平均低于对照组,血清超氧化物歧化酶、谷胱甘肽过氧化物酶水平高于对照组,差异有统计学意义(P均<0.05).观察组胰岛素用量(36.29±3.48)U/d少于对照组,观察组空腹血糖达标时间(6.22±1.17)d短于对照组,差异有统计学意义(t=3.562、3.250,P均<0.05).结论 对老年2型糖尿病患者实施恩格列净联合胰岛素治疗可以减少胰岛素用量,调节胰岛素抵抗指数,改善其抗氧化能力,有效且快速控制血糖.
Objective To explore the effects of combination of insulin and empagliflozin on antioxidant capacity,blood glucose control and insulin dosage in elderly patients with type 2 diabetes mellitus.Methods The clinical data of ninety-two elderly patients with type 2 diabetes mellitus admitted to General Global Midway Xianyang Hospital from February 2022 to February 2024 were retrospectively selected.According to different treatment methods,they were di-vided into observation group(insulin combined with empagliflozin)and control group(insulin treatment),with forty-six cases each.Homeostasis model assessment-IR(HOMA-IR),blood glucose control,insulin dosage,antioxidant capac-ity and fasting blood glucose compliance time were compared between the two groups.Results There were no signifi-cant differences in fasting plasma glucose,HOMA-IR and 2-hour postprandial plasma glucose levels between the two groups before treatment(all P>0.05).After treatment,fasting plasma glucose,HOMA-IR and 2-hour postprandial plasma glucose levels in the observation group were lower than those in the control group,and the differences were sta-tistically significant(all P<0.05).There were no significant differences in the levels of glutathione peroxidase,malondi-aldehyde,reactive oxygen species and serum superoxide dismutase between two groups before treatment(all P>0.05).After treatment,the levels of malondialdehyde and active oxygen in observation group were lower than those in control group,and the levels of superoxide dismutase and glutathione peroxidase in serum were higher than those in control group,and the differences were statistically significant(all P<0.05).The insulin dosage of the observation group(36.29±3.48)U/d was lower than that of the control group,and the fasting plasma glucose standard time of the observa-tion group(6.22±1.17)d was less than that of the control group,and the differences were statistically significant(t=3.562,3.250,both P<0.05).Conclusion Empagliflozin combined with insulin therapy in elderly patients with type 2 diabetes mellitus can reduce insulin dosage,adjust insulin resistance index,improve antioxidant capacity,and effec-tively and quickly control blood glucose.
