基于血糖变异探究门冬胰岛素联合地特胰岛素治疗老年T2DM患者的价值

Exploring the Value of Insulin Aspartate Combined with Insulin Detecin in the Treatment of Elderly T2DM Patients Based on Blood Glucose Variabil-ity

目的 基于血糖变异探究门冬胰岛素(insulin aspart,IA)联合地特胰岛素(insulin detemir,ID)治疗老年2型糖尿病(type 2 diabetes mellitus,T2DM)的价值.方法 选取2021年10月—2022年11月福建医科大学附属漳州市医院收治的70例老年T2DM患者为研究对象,依据随机数表法分为两组,各35例.联合组接受IA联合ID治疗,对照组接受IA治疗.比较两组患者血糖水平、体重指数(body mass index,BMI)、血糖变异性以及血糖达标时间.结果 治疗3个月后,联合组BMI水平、空腹血糖分别为(25.11±2.23)kg/m2、(6.21±0.53)mmol/L低于对照组的(26.57±2.41)kg/m2、(6.57±0.51)mmol/L,差异有统计学意义(P均<0.05).治疗后,联合组最大血糖波动幅度、餐后血糖波动幅度、空腹血糖变异系数、平均血糖波动幅度水平分别为(4.22±0.88)mmol/L、(2.66±0.31)mmol/L、9.84±2.51、(3.63±0.24)mmol/L,均显著低于对照组的(6.31±1.12)mmol/L、(3.57±0.42)mmol/L、14.27±5.48、(4.27±0.22)mmol/L,差异有统计学意义(P均<0.05).治疗后,联合组血糖达标时间为(3.87±1.22)d短于对照组的(5.34±1.76)d,差异有统计学意义(P<0.05).结论 老年T2DM患者采用IA联合ID治疗可控制其血糖水平,同时稳定血糖变异性并显著缩短血糖达标时间.
Objective To explore the value of insulin aspart(IA)combined with insulin detemir(ID)in the treatment of elderly type 2 diabetes mellitus(T2DM)based on blood glucose variability.Methods A total of 70 elderly patients with T2DM admitted to Zhangzhou Affiliated Hospital of Fujian Medical University from October 2021 to November 2022 were selected as the research objects.According to the random number table method,they were divided into two groups,with 35 cases in each group.The combination group was treated with IA combined with ID,and the control group was treated with IA.The blood glucose level,body mass index(BMI),blood glucose variability,and blood glu-cose control time were compared between the two groups.Results After 3 months of treatment,the BMI levels and fast-ing blood glucose in the combination group were(25.11±2.23)kg/m2 and(6.21±0.53)mmol/L,which were lower than those in the control group(26.57±2.41)kg/m2 and(6.57±0.51)mmol/L,and the differences were statistically signifi-cant(both P<0.05).After treatment,the maximum blood glucose fluctuation amplitude,postprandial blood glucose fluctuation amplitude,fasting blood glucose variation coefficient and average blood glucose fluctuation amplitude in the combined group were(4.22±0.88)mmol/L,(2.66±0.31)mmol/L,9.84±2.51 and(3.63±0.24)mmol/L significantly lower than those in the control group(6.31±1.12)mmol/L,(3.57±0.42)mmol/L,14.27±5.48 and(4.27±0.22)mmol/L,and the differences were statistically significant(all P<0.05).After treatment,the time for blood glucose to reach the standard in the combination group was(3.87±1.22)d shorter than that in the control group(5.34±1.76)d,and the dif-ference was statistically significant(P<0.05).Conclusion The combination of IA and ID therapy in elderly T2DM pa-tients can control the blood glucose level,stabilize the blood glucose variability and significantly shorten the blood glu-cose compliance time.
