Diabetes mellitus belongs to our country and worldwide incidence of chronic diseases,high hospitalization of diabetes patients by medical staff comprehensive care,but after discharge,patients return to the family,oneself and caregivers are lack of nursing skills,difficult to provide continuous care for patients,readmission risk increased ac-cordingly.It can be seen that continuous nursing is an important part of the out-of-hospital health management of dia-betic patients.At present,the content of continuous nursing applied for diabetes in our country is not perfect,and it is critical to explore the continuous nursing in line with the actual nursing needs of diabetic patients in our country.Therefore,this paper combines the concept of continuous nursing and the development status,and expounds the pros-pect and clinical application effect of continuous nursing,in order to provide reference value for out-of-hospital nurs-ing for diabetic patients and provide the best care for patients.
