摘要
目的 探究运用地特胰岛素联合门冬胰岛素方案治疗妊娠期糖尿病的效果.方法 选取2021年12月—2023年12月漳州市第二医院收治的90例妊娠期糖尿病患者为研究对象,根据不同治疗方法分为对照组、观察组,各45例.对照组行门冬胰岛素治疗,观察组行地特胰岛素联合门冬胰岛素方案治疗.对比两组血糖达标率、血糖情况(空腹血糖、餐后2 h血糖、糖化血红蛋白)、新生儿不良结局(新生儿低血糖症、巨大儿、新生儿窒息)以及产妇不良妊娠结局(胎盘早剥、胎膜早破、早产)发生率.结果 治疗3个月后,观察组血糖达标率高于对照组,差异有统计学意义(P＜0.05).与治疗前相比,治疗3个月后两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均降低,且观察组低于对照组,差异有统计学意义(P均＜0.05).观察组新生儿不良结局发生率、产妇不良妊娠结局发生率均低于对照组,差异有统计学意义(P均＜0.05).结论 临床在治疗妊娠期糖尿病时采取地特胰岛素联合门冬胰岛素治疗方案效果明确,能加快血糖达标,稳定血糖水平,改善新生儿与产妇不良结局.
Abstract
Objective To explore the effect of insulin detemir combined with insulin aspart in the treatment of gesta-tional diabetes mellitus.Methods A total of 90 patients with gestational diabetes mellitus admitted to Zhangzhou Sec-ond Hospital from December 2021 to December 2023 were selected as the research objects.They were divided into control group and observation group according to different treatment methods,with 45 cases in each group.The control group was treated with insulin aspart,and the observation group was treated with insulin detemir combined with insu-lin aspart.The blood glucose compliance rate,blood glucose(fasting blood glucose,2-hour postprandial blood glu-cose,glycated hemoglobin A1c),neonatal adverse outcomes(neonatal hypoglycemia,macrosomia,neonatal asphyxia)and maternal adverse pregnancy outcomes(placental abruption,premature rupture of membranes,premature delivery)were compared between the two groups.Results After 3 months of treatment,the blood glucose compliance rate of the observation group was higher than that of the control group,and the difference was statistically significant(P＜0.05).Compared with before treatment,the levels of fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the two groups decreased after 3 months of treatment,and the levels in the observation group were lower than those in the control group,the differences were statistically significant(all P＜0.05).The incidence of neona-tal adverse outcomes and maternal adverse pregnancy outcomes in the observation group were lower than those in the control group,and the differences were statistically significant(both P＜0.05).Conclusion The clinical effect of insulin detemir combined with insulin aspart in the treatment of gestational diabetes mellitus is clear,which can accelerate the blood glucose standard,stabilize the blood glucose level,and improve the adverse outcome of newborns and partu-rients.
