Objective To analyze the effect of diet and exercise therapy combined with insulin aspart and insulin de-temir in the treatment of gestational diabetes mellitus,and to analyze the difference of blood glucose indexes between the groups before and after treatment.Methods A total of 80 patients with gestational diabetes mellitus admitted to Fuzhou Jiahe Jiamei Hospital from April 2022 to April 2023 were selected as the research objects.According to differ-ent treatment methods,they were divided into control group and observation group,with 40 cases in each group.Both groups were treated with diet and exercise.The control group was treated with insulin aspart,and the observation group was treated with insulin detemir on the basis of the control group.The blood glucose indexes[fasting plasma glu-cose(FPG),2-hour postprandial plasma glucose(2 hPG),glycated hemoglobin A1c(HbA1c)],serum indexes[homo-cysteine(HCY),cystatin C(CysC)],cesarean section rate,and incidence of adverse outcomes(neonatal respiratory dis-tress,macrosomia,neonatal hypoglycemia)were compared between the two groups.Results The levels of FPG,2 hPG and HbA1c in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The levels of HCY and CysC in the observation group were lower than those in the control group,and the differences were statistically significant(both P<0.05).The cesarean section rate of the observation group was 5.00%(2/40),which was lower than 25.00%(10/40)of the control group,and the difference was statistically significant(χ2=5.186,P=0.023).The incidence of neonatal adverse outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Diet and exercise therapy combined with insulin aspart and insulin detemir in the treatment of gestational diabetes is conducive to con-trolling blood glucose,regulating serum indicators and improving maternal and infant outcomes.
