摘要
目的 探究不同病情严重程度糖尿病肾病与尿白蛋白排泄率(urinary albumine xcretion,UAE)、血清β2微球蛋白(β2-microglobulin,β2-MG)、胱抑素C(cystatin C,CysC)、尿微量白蛋白与尿肌酐比值(albumin/cre-atinine ratio,UACR)的相关性.方法 选取2020年11月—2023年12月福建省三明市明溪县总医院收治的174例2型糖尿病肾病患者作为研究对象,依据糖尿病肾病分期分为Ⅰ期组(n=40)、Ⅱ期组(n=37)、Ⅲ期组(n=32)、Ⅳ期组(n=35)、Ⅴ期组(n=30).选择同期在本院体检的60例健康人作为对照组.比较以上所有组的血清β2-MG、CysC、UACR、UAE水平,并对不同病情严重程度与UAE、血清β2-MG、CysC、UACR进行相关性分析.结果 Ⅰ期组、Ⅱ期组、Ⅲ期组、Ⅳ期组、Ⅴ期组与对照组血清β2-MG、CysC、UACR、UAE水平比较,差异有统计学意义(P均<0.05);Ⅰ期组与Ⅱ期组、Ⅲ期组、Ⅳ期组、Ⅴ期组血清β2-MG、CysC、UACR、UAE水平相比,差异有统计学意义(P均<0.05);Ⅱ期组与Ⅲ期组、Ⅳ期组、Ⅴ期组血清β2-MG、CysC、UACR、UAE水平相比,差异有统计学意义(P均<0.05).Pearson相关性分析结果显示,UAE、血清β2-MG、CysC、UACR水平与糖尿病肾病病情严重程度呈正相关(P均<0.05).结论 不同病情严重程度糖尿病肾病患者的UAE、血清β2-MG、CysC、UACR具有相关性,疾病越严重,UAE、血清β2-MG、CysC、UACR水平就越高.
Abstract
Objective To explore the correlation between different severity of diabetic nephropathy and urinary albu-min excretion rate(UAE),serum β2-microglobulin(β2-MG),cystatin C(CysC),urinary albumin/creatinine ratio(UACR).Methods A total of 174 patients with type 2 diabetic nephropathy admitted to Mingxi General Hospital of Sanming in Fujian Province from November 2020 to December 2023 were selected as the research objects.According to the stage of diabetic nephropathy,the patients were divided into stage I group(n=40),stage Ⅱ group(n=37),stageⅢ group(n=32),stage Ⅳ group(n=35)and stage V group(n=30).Sixty healthy people who underwent physical exami-nation in our hospital during the same period were selected as the control group.The levels of serum β2-MG,CysC,UACR and UAE in all groups were compared,and the correlation between the severity of different diseases and UAE,serum serum β2-MG,CysC and UACR was analyzed.Results There were statistically significant differences in the levels of β2-MG,CysC,UACR and UAE between stage I group,stage Ⅱ group,stage Ⅲ group,stage Ⅳ group,stage V group and control group(all P<0.05).The levels of serum β2-MG,CysC,UACR and UAE in stage I group were sta-tistically differences from those in stage Ⅱ group,stage Ⅲ group,stage Ⅳ group and stage V group(all P<0.05).There were statistically significant differences in the levels of serum β2-MG,CysC,UACR and UAE between stageⅡ group and stage Ⅲ group,stage Ⅳ group and stage V group(all P<0.05).Pearson correlation analysis showed that UAE,serum β 2-MG,CysC and UACR levels were positively correlated with the severity of diabetic nephrapathy(all P<0.05).Conclusion There was a correlation between UAE,serum β2-MG,CysC and UACR in patients with different severity of diabetic nephropathy.The more severe the disease,the higher the levels of UAE,serum β2-MG,CysC and UACR.
