摘要

目的 探究门冬胰岛素与地特胰岛素联合用于妊娠期糖尿病(gestational diabetes mellitus,GDM)孕妇糖代谢控制中对妊娠结局的影响.方法 回顾性选取2023年4月—2024年2月泉州市第一医院收治的98例GDM孕妇的临床资料,按照不同控糖方式分为两组,各49例.对照组单独行门冬胰岛素治疗,研究组行门冬胰岛素与地特胰岛素联合治疗,比较不同控糖方式对不良妊娠结局(早产、流产、巨大儿、低出生体重儿)、糖代谢水平(空腹血糖、餐后2 h血糖、糖化血红蛋白)及胰岛功能(空腹胰岛素、胰岛素抵抗指数)的影响.结果 两组不良妊娠结局发生率对比,差异无统计学意义(P>0.05).治疗前,两组糖代谢、胰岛功能水平对比,差异无统计学意义(P均>0.05);分娩后,研究组糖代谢水平、胰岛功能均优于对照组,差异有统计学意义(P均<0.05).结论 门冬胰岛素对GDM孕妇实施有效控糖基础上联用地特胰岛素,不会增加不良妊娠结局发生风险,糖代谢水平及胰岛功能均有所改善.

Abstract

Objective To explore the effect of insulin aspart combined with insulin detemir on pregnancy outcome in glucose metabolism control of pregnant women with gestational diabetes mellitus(GDM).Methods The clinical data of 98 pregnant women with GDM admitted to Quanzhou First Hospital from April 2023 to February 2024 were retrospec-tively selected.According to different glucose control methods,they were divided into two groups,49 cases in each group.The control group was treated with insulin aspart alone,and the study group was treated with insulin aspart combined with insulin detemir.The effects of different glucose control methods on adverse pregnancy outcome(prema-ture delivery,abortion,macrosomia,low birth weight infants),glucose metabolism level(fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin A1c)and islet function(fasting insulin,insulin resistance index)were compared.Results There was no significant difference in the incidence of adverse pregnancy outcomes between the two groups(P>0.05).Before treatment,there were no significant differences in glucose metabolism and islet function between the two groups(all P>0.05).After delivery,the levels of glucose metabolism and islet function in the study group were better than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion Insulin aspart combined with insulin detemir on the basis of effective glucose control in GDM pregnant women will not increase the risk of adverse pregnancy outcomes,and the level of glucose metabolism and islet func-tion are improved.
