Effect of Insulin Glargine on Blood Glucose Control in Elderly Patients with Diabetes Mellitus Complicated with Severe Pneumonia in ICU

目的 探究甘精胰岛素控制重症加强护理病房(intensive care unit,ICU)老年糖尿病合并重症肺炎患者血糖水平的作用.方法 回顾性选取2022年9月—2023年12月泉州市第一医院收治的96例糖尿病合并重症肺炎患者的临床资料,根据治疗方法的不同分为两组,各48例.两组均对症实施肺炎治疗,对照组使用常规胰岛素治疗,研究组使用甘精胰岛素治疗.对比两组血糖水平、治疗情况和预后指标.结果 治疗后,研究组空腹血糖、餐后2 h血糖水平均低于对照组,差异有统计学意义(P均<0.05).研究组血糖达标时间短于对照组,胰岛素用量少于对照组,差异有统计学意义(P均<0.05).两组28 d内均未出现死亡病例;研究组低血糖发生率低于对照组,差异有统计学意义(P<0.05).结论 甘精胰岛素在治疗ICU老年糖尿病合并重症肺炎患者时,能够显著控制血糖水平,促进血糖达标,降低胰岛素用量,并减少低血糖发生率.
Objective To explore the effect of insulin glargine on blood glucose level in elderly patients with diabetes mellitus complicated with severe pneumonia in intensive care unit(ICU).Methods The clinical data of 96 patients with diabetes mellitus complicated with severe pneumonia admitted to Quanzhou First Hospital from September 2022 to December 2023 were retrospectively selected and divided into two groups according to different treatment methods,with 48 cases in each group.Both groups were symptomatically treated with pneumonia.The control group was treated with conventional insulin,and the study group was treated with insulin glargine.The blood glucose levels,treatment and prognostic indicators were compared between the two groups.Results After treatment,the levels of fasting blood glucose and 2-hour postprandial blood glucose in the study group were lower than those in the control group,and the differences were statistically significant(both P<0.05).The time of blood glucose reaching the standard in the study group was shorter than that in the control group,and the dosage of insulin was less than that in the control group,the differences were statistically significant(both P<0.05).No death occurred in the two groups within 28 days.The inci-dence of hypoglycemia in the study group was lower than that in the control group,the difference was statistically sig-nificant(P<0.05).Conclusion Insulin glargine in the treatment of ICU elderly patients with diabetes mellitus compli-cated with severe pneumonia can significantly control blood glucose levels,promote blood glucose compliance,reduce insulin dosage,and reduce the incidence of hypoglycemia.
