自拟益肾降糖方联合西药治疗糖尿病肾病的疗效及对氧化应激指标、血管内皮活性的影响

摘要
目的 探讨自拟益肾降糖方联合西药治疗糖尿病肾病的临床疗效.方法 选取2020年4月—2024年3月齐河县中医院收治的92例糖尿病肾病患者为研究对象,以治疗方法的不同分为对照组(46例,实施西药治疗)和观察组(46例,实施自拟益肾降糖方联合西药治疗).比较两组血糖水平、氧化应激指标、血管内皮活性以及肾功能.结果 治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组,差异有统计学意义(P均<0.05).治疗后,观察组谷胱甘肽过氧化物酶、超氧化物歧化酶水平均高于对照组,丙二醛水平低于对照组,差异有统计学意义(P均<0.05).治疗后,观察组一氧化氮水平高于对照组,血管内皮生长因子、血肌酐、尿素氮水平均低于对照组,差异有统计学意义(P均<0.05).结论 糖尿病肾病患者应用自拟益肾降糖方联合西药治疗的效果显著,能有效控制患者的血糖水平,发挥抗氧化应激作用,改善其血管内皮功能及肾功能.
Abstract
Objective To explore the clinical efficacy of self-made Yishen Jiangtang Prescription combined with Western medicine in the treatment of diabetic nephropathy.Methods A total of 92 patients with diabetic nephropathy admitted to Qihe County Hospital of Traditional Chinese Medicine from April 2020 to March 2024 were selected as the research objects.According to different treatment methods,they were divided into control group(46 cases,treated with Western medicine)and observation group(46 cases,treated with self-made Yishen Jiangtang Prescription com-bined with Western medicine).The blood glucose levels,oxidative stress indexes,vascular endothelial activity and re-nal function were compared between the two groups.Results After treatment,the levels of fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the levels of glutathione peroxidase and superoxide dismutase in the observation group were higher than those in the control group,and the level of malondialdehyde was lower than that in the control group,the differences were statistically significant(all P<0.05).After treatment,the level of nitric oxide in the observation group was higher than that in the control group,and the levels of vascular endothelial growth factor,serum creatinine and urea nitrogen were lower than those in the con-trol group,the differences were statistically significant(all P<0.05).Conclusion The effect of self-made Yishen Jiang-tang Prescription combined with Western medicine in patients with diabetic nephropathy is significant.It can effec-tively control the blood glucose levels of patients,play an anti-oxidative stress role,and improve their vascular endo-thelial function and renal function.
