糖尿病肾病患者应用二甲双胍联合胰岛素强化治疗的效果分析

摘要
目的 探究二甲双胍联合胰岛素强化治疗在糖尿病肾病(diabetic nephropathy,DN)患者中的应用效果.方法 回顾性选取2022年2月—2024年2月南方医科大学南方医院白云分院收治的120例DN患者的临床资料,根据不同治疗方法分为对照组和研究组,各60例.对照组采用二甲双胍治疗,研究组在对照组的基础上采用甘精胰岛素联合门冬胰岛素强化治疗.比较两组临床疗效、肾功能指标、糖代谢指标.结果 研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05).治疗后,研究组尿素氮、肌酐、尿白蛋白排泄率、空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组,差异有统计学意义(P均<0.05).结论 DN患者应用二甲双胍联合胰岛素强化治疗的临床疗效显著,可改善患者的肾功能及糖代谢指标.
Abstract
Objective To explore the application effect of metformin combined with intensive insulin therapy in pa-tients with diabetic nephropathy(DN).Methods The clinical data of 120 patients with DN admitted to Southern Medi-cal University Southern Hospital Baiyun Branch from February 2022 to February 2024 were retrospectively selected.According to different treatment methods,they were divided into control group and study group,60 cases in each group.The control group was treated with metformin,and the study group was treated with insulin glargine combined with insulin aspart on the basis of the control group.The clinical efficacy,renal function index and glucose metabo-lism index were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the levels of urea nitrogen,creatinine,urinary albumin excretion rate,fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the study group were lower than those in the control group,and the differences were statis-tically significant(all P<0.05).Conclusion The clinical efficacy of metformin combined with intensive insulin therapy in patients with DN is significant,which can improve the renal function and glucose metabolism index of patients.
