运动对骨量减少老年人影响效果的Meta分析

目的:通过评价分析运动干预对骨量减少老年人的影响效果,为骨量减少老年人的运动实践干预提供证据支持,并探讨最佳运动干预方案.方法:通过检索PubMed、Web of Science、Cochrane Library、Em-base、中国知网、万方等6个数据库,收集相关研究的国内外文献,并通过使用RevMan5.4.1软件进行Meta分析.结果:共纳入14篇文献,Meta分析结果显示:运动对骨密度T值[MD=0.25,95% CI(0.13,0.37),P<0.0001]、骨密度[SMD=0.30,95% CI(0.05,0.54),P=0.02]、生活质量[MD=-6.33,95% CI(-8.43,-4.24),P<0.00001](QUALEFFO 评分)[MD=2.18,95% CI(0.92,3.43),P=0.0007](SF-36 量表)等的影响具有显著差异.结论:运动干预可以改善骨量减少老年人的身体功能、预防骨量减少、延缓骨质流失;预达到有效运动干预.建议骨量减少老年人可以进行运动周期为24周(或>12周)、运动频率≥3次/周、时间60min或者30-60min的有氧运动、阻力运动或敏捷运动.
The Impact and Effectiveness of Exercise on Bone Mass Reduction in Older Adults:A Meta-Analysis
OBJECTIVE:This study aims to assess and analyze the effects of exercise interventions on bone mass reduction in older adults.By providing evidence-based support for exercise interventions in this population,it also aims to explore optimal exercise intervention strategies.METHOD:A comprehensive search of six databases,including PubMed,Web of Science,Cochrane Library,Embase,CNKI,and Wanfang,was conducted to collect relevant domestic and international literature.A meta-analysis was performed using RevMan 5.4.1 software.RESULTS:Fourteen previous studies were included in the analysis.The meta-analysis results revealed significant differences in the impact of exercise on bone density T-score[MD=0.25,95%CI(0.13,0.37),P<0.0001],bone density[SMD=0.30,95% CI(0.05,0.54),P=0.02],quality of life(QUALEFFO score)[MD=-6.33,95% CI(-8.43,-4.24),P<0.00001],and SF-36 scale[MD=2.18,95% CI(0.92,3.43),P=0.0007].CONCLUSION:Exercise interventions can improve physical function,prevent bone mass reduction,and delay bone loss in older adults.For effective exercise interventions,it is recommended that older adults with bone mass reduction engage in aerobic exercises,resistance training,or agility exercises for a duration of 24 weeks(or>12 weeks),with a frequency of ≥3 times per week,and a session duration of either 60 minutes or 30-60 minutes.
