Relationship between antibody typing of Helicobacter pylori infection and severity of chronic gastritis and mucosal histological changes
Objective To investigate the relationship between Helicobacter pylori(H.pylori)infection antibody typ-ing and the severity of chronic gastritis and mucosal histological changes.Methods A total of 507 chronic gastritis pa-tients treated in our hospital from Jan.2019 to Apr.2022 were selected as study objects.According to the severity of chronic gastritis,the patients were divided into mild group(n=205),moderate group(n=180)and severe group(n=122).After 1∶1∶1 propensity score matching(PSM),100 patients were selected from each group to compare the gener-al data,clinical indicators affecting the occurrence of severe chronic gastritis,the correlation analysis between the sever-ity of chronic gastritis and the change of mucosal histology and the typing of H.pylori antibody in the three groups.Mult-ivariate Logistic regression analysis was used to identify risk factors affecting the occurrence of severe chronic gastritis.A structural equation model was constructed to analyze the effects of influencing factors.Spearman correlation test was used to analyze the correlation between chronic gastritis severity and mucosal histological changes and H.pylori antibody typ-ing.Results H.pylori antibody typing,gastric mucosal lesions,degree of intestinal metaplasia and gastric mucosa atro-phy were all independent risk factors affecting the occurrence of severe chronic gastritis(P<0.05).The structural equa-tion model was well adapted and could reflect the relationship between various factors and the occurrence of severe chro-nic gastritis.The severity of chronic gastritis and the degree of gastric mucosa atrophy were significantly positively corre-lated with H.pylori-Ⅰ infection(P<0.05),while the negative H.pylori antibody was significantly negatively correlated with the severity of chronic gastritis and the degree of gastric mucosa atrophy(P<0.05).Conclusion H.pylori infec-tion antibody typing has high clinical application value in the severity of chronic gastritis and the degree of gastric mucosa atrophy,and is closely related to the severity of chronic gastritis patients and the histological changes of gastric mucosa.
