Risk factors for acute lung injury and acute respiratory distress syndrome in severe acute pancreatitis:a Meta-analysis
Objective To systematically analyze the risk factors of acute lung injury(ALI)and acute respiratory distress syndrome(ARDS)in severe acute pancreatitis(SAP).Methods Chinese and English databases were searched for articles about risk factors of ALI or ARDS in SAP.According to the inclusion and exclusion criteria,two researchers independently conducted literature screening,data extraction and quality assessment.RevMan 5.4 and Stata 17.0 software were used to per-form a Meta-analysis of the included data.Results A total of 23 studies involving 5 413 patients were included.The results of Meta-analysis showed that smoking history(P=0.040),hyperlipidemia(P<0.001),BMI(P=0.050),blood glucose(P<0.001),heart rate(P=0.004),respiratory rate(P=0.009),white blood cell count(P=0.001),neutrophil count(P<0.001),CRP(P<0.001),oxygenation index(P<0.001),Ca2+(P=0.020),Ranson score(P<0.001),APACHE Ⅱ score(P<0.001),CTSI score(P=0.002),murray lung injury score(P<0.001),systemic inflammatory response syndrome(SIRS)(P=0.020),multiple organ dysfunction syndrome(MODS)(P<0.001),pleural effusion(P=0.001),infection(P=0.001)were risk factors for ALI and ARDS in patients with SAP.Conclusion Clinicians should pay great attention to the risk factors of acute lung injury and acute respiratory distress syndrome in patients with severe acute pancreatitis,identify high-risk patients early,and reduce their incidence and mortality.
