急性胰腺炎合并代谢综合征相关危险因素分析

目的 探讨代谢综合征(metabolic syndrome,MS)及相关组分合并急性胰腺炎(acute pancreatitis,AP)相关危险因素.方法 收集 2020 年 1 月至 2023 年 1 月武汉大学人民医院收治的 182 例 AP 患者资料,根据 MS诊断标准将患者分为 MS 组(n=99)和非MS组(n=83),通过单、多因素 Logistic回归分析及 ROC曲线分析AP合并MS患者的危险因素.结果 AP 患者 MS的发生率为 54%(99/182),两组患者在尿酸、血糖、电解质、血脂及血常规等相关指标方面,差异有统计学意义(P<0.05).ROC 曲线显示上述独立预测因素(心率、空腹血糖、血红蛋白)联合预测 AP 合并 MS的 AUC 为 0.830.结论 AP 患者患高血压、糖尿病、心率、空腹血糖、血红蛋白是 AP 合并 MS发生的独立危险因素.心率、血糖、血红蛋白联合检测对 AP 发生 MS具有一定的预测价值,或是早期识别MS合并 AP 患者有效的预测指标.
Analysis of risk factors associated with acute pancreatitis combined with metabolic syndrome
Objective To explore the risk factors associated with metabolic syndrome(MS)and related components combined with acute pancreatitis(AP).Methods The data of 182 patients with acute pancreatitis admitted to the Renmin Hospital of Wuhan University from Jan.2020 to Jan.2023 were collected,and the patients were divided into the MS group(n=99)and the non-MS group(n=83)according to the diagnostic criteria of MS,and the risk factors associated with AP combined with related components were analyzed by single-and multifactorial Logistic regression ana-lyses and the ROC curve.The risk factors of patients with AP combined with MS were analyzed by single and multifacto-rial Logistic regression analysis and ROC curve.Results The prevalence of MS in AP patients was 54% (99/182),and there were significant differences between the two groups in terms of uric acid,blood glucose,electrolytes,lipids,and routine blood tests(P<0.05).The ROC curve showed that the AUC for the combination of the above independent predictors(heart rate,fasting glucose and hemoglobin)to predict combined MS in AP was 0.830.The ROC curve showed that the AUC was 0.830.Conclusion The presence of hypertension,diabetes mellitus,heart rate,fasting glu-cose and hemoglobin in AP patients are independent risk factors for the development of MS in combination with AP.The combined test of heart rate,blood glucose and hemoglobin levels has a certain predictive value for the occurrence of MS in AP or is an effective predictor for early identification of combined MS in patients with AP.
