Effectiveness and safety of integrated traditional Chinese medicine and Western medicine therapy in eradicating Helicobacter pylori infection in the elderly:system-atic review and Meta-analysis
Objective To evaluate the efficacy and safety of integrated traditional Chinese medicine(TCM)and Western medicine treatment regimens in elderly patients with Helicobacter pylori(H.pylori)infection,aiming to provide evidence-based support for optimizing integrated TCM and Western medicine treatment plans for eradicating H.pylori in the elderly.Methods A comprehensive search was conducted in PubMed,Cochrane Library,Science Direct,Web of Science,CNKI,VIP,Wanfang Database,CBM and Yiigle database up to Feb.1st,2024,for clinical trials involving H.pylori-infected individuals aged 60 years old and above.Meta-analysis was performed using RevMan 5.4.1,with dif-ferent models selected based on heterogeneity assessment results.Funnel plots were used to assess bias risk.Results A total of 15 studies met the inclusion criteria,with a combined sample size of 1 468 cases.The Meta-analysis revealed that TCM integrated therapy showed reliable eradication outcome compared to using Western medicine alone(RR=1.20,95%CI:1.14-1.25,P<0.00001),although it did not significantly improve the eradication rate of relapse(RR=2.50,95%CI:0.82-7.63,P=0.11).Evaluating the safety of the integrated therapy based on adverse event occurrence indicated that it was safer than Western medicine group(RR=0.56,95%CI:0.40-0.80,P=0.001),suggesting that the TCM integrated therapy was safe and reliable for implementation.Conclusion Among elderly pa-tients infected with H.pylori who face multiple challenges in H.pylori eradication.TCM integrated therapy shows positive results on effectiveness and security for the eradication of H.pylori in elderly patients,offering a promising alternative therapy option that combines the benefits of both medical traditions while addressing potential limitations such as drug tolerance and adverse effects associated with antibiotic use.Moreover,integrated regimen can significantly alleviate the digestive symptoms in H.pylori eradication,thereby substantially enhancing the treatment experience for the elderly re-garding eradication treatment.Given its promising outcomes,it is worth exploring TCM integrated therapy in broader patient populations with various complicating factors through high-quality,large-scale clinical trials to gather robust evidence-based medical support.
