Endoscopic and pathological features and risk factor of synchronous multiple primary gastrointestinal cancer in early esophageal cancer
Objective To investigate the clinical and endoscopic pathological characteristics of synchronous multiple primary gastrointestinal cancer in patients with early esophageal cancer and analyze the independent risk factors for the occurrence of synchronous multiple primary gastrointestinal cancers.Methods Endoscopic and clinicopathological fea-tures of 321 patients diagnosed as early esophageal cancer were retrospectively collected from 2013 to 2022.Univariate and multivariate Logistic regression analysis was used to select the independent risk factors affecting synchronous multiple primary gastrointestinal cancers.Results Among 321 cases of early esophageal cancer,a total of 17 cases(5.3%)were found to have synchronous multiple primary gastrointestinal cancers.Gastric cancer was the most common type(13 cases,76.5%),with cardia cancer being predominant(8 cases,47.1%).The majority of these cases(10 cases,76.9%)presented as early-stage lesions.The independent risk factors for developing synchronous multiple primary gas-trointestinal cancer in patients with early esophageal cancer were lower lesion location(OR=4.10,95%CI:1.26-13.31,P=0.019)and multiple lesions in the esophagus(OR=4.60,95%CI:1.60-13.21,P=0.005).Conclusion In order to avoid missing multiple primary cancers and achieve early diagnosis and treatment,physicians should perform more detailed gastrointestinal examinations on early esophageal cancer patients who have risk factors for synchronous multiple primary gastrointestinal cancers.
