摘要
目的:探讨双侧肾上腺腺瘤行肾上腺静脉采血患者单侧采血失败后,对侧抑制指数在优势侧诊断中的应用价值.方法:总结2022年8月至2024年2月于唐山市工人医院因双侧肾上腺腺瘤行肾上腺静脉采血患者的基本资料,回顾性分析其中5例因单侧肾上腺静脉采血失败、通过计算对侧抑制指数确定优势侧患者的基本信息(性别、年龄、高血压病史、是否存在低钾血症及入院血压)、检验及病理学资料,并随访治疗效果.结果:5例患者均为右侧肾上腺静脉采血失败[侧别指数(LI)<2],通过左侧对侧抑制指数确定手术侧别,术后均取得良好的临床疗效,说明对侧抑制指数可为单侧肾上腺静脉采血失败患者优势侧的确定提供参考价值.结论:对侧抑制指数在双侧肾上腺腺瘤优势侧的确定中具有良好的临床应用价值,尤其在单侧肾上腺静脉采血失败时可作为优势侧选择的参考.
Abstract
Objective:To explore the application value of the contralateral suppression index(CSI)in determin-ing the dominant side in patients with bilateral adrenal adenomas who experience failure of unilateral adrenal venous sampling(AVS).Methods:We summarized the basic data of patients who underwent adrenal venous sampling for bilateral adrenal adenomas at Tangshan Workers'Hospital from August 2022 to February 2024.A retrospective anal-ysis was conducted on 5 patients who failed unilateral adrenal venous sampling and determined the dominant side us-ing the contralateral suppression index.The basic information of these patients(sex,age,hypertension history,pres-ence of hypokalemia,and admission blood pressure),laboratory findings,and pathological data were reviewed.Fol-low-up data on treatment outcomes were also collected.Results:In all 5 patients,right-sided adrenal venous sam-pling failed(lateralization index[LI]<2).The dominant side was determined using the left-sided contralateral sup-pression index,and all patients achieved favorable clinical outcomes postoperatively.This indicates that the contralat-eral suppression index can provide valuable information for determining the dominant side in cases where unilateral ad-renal venous sampling fails.Conclusion:The contralateral suppression index has significant clinical application value in determining the dominant side in bilateral adrenal adenomas,particularly when unilateral adrenal venous sampling fails.It can serve as a useful reference for selecting the surgical side.
