摘要
目的:探讨经皮肾镜联合铥激光肾囊肿去顶减压术治疗单纯性肾囊肿的疗效和安全性.方法:选取2021年1月至2023年12月在重庆医科大学附属永川医院接受治疗的56例单纯性肾囊肿患者,将其按照随机数字表法分为实验组和对照组,每组28例.实验组行经皮肾镜联合铥激光肾囊肿去顶减压术,对照组行普通腹腔镜肾囊肿去顶减压术.观察两组患者的一般情况、手术时间、术中出血、术后视觉模拟评分法(VAS)评分、围术期生化指标、引流管留置时间及住院时间.结果:两组患者一般情况比较差异无统计学意义,实验组手术时间、术后引流管留置时间、住院时间较对照组缩短,术中出血量、VAS评分和C-反应蛋白(CRP)较对照组低,差异均有统计学意义(P均<0.05).术前和术后血肌酐、血浆渗透压(Posm)、血白细胞计数(WBC)两组间比较差异均无统计学意义(P均>0.05).两组患者术后并发症发生率比较差异无统计学意义(P>0.05).结论:经皮肾镜联合铥激光治疗单纯性肾囊肿具有创伤小、手术时间短、出血少和恢复快等优势,是一种新的、安全有效的治疗方式.
Abstract
Objective:To explore the efficacy and safety of percutaneous nephrolithotomy combined with thuli-um laser cystectomy for the treatment of simple renal cysts.Methods:A total of 56 patients with simple renal cysts who were treated at Chongqing Medical University Affiliated Yongchuan Hospital from January 2021 to December 2023 were selected and randomly divided into two groups using a random number table:the experimental group(28 cases)and the control group(28 cases).The experimental group underwent percutaneous nephrolithotomy combined with thulium laser cystectomy,while the control group received conventional laparoscopic cystectomy.The general conditions of the patients,operation time,intraoperative blood loss,postoperative visual analog scale(VAS)score,perioperative biochemical parameters,duration of drainage tube placement,and length of hospital stay were observed and compared between the two groups.Results:There were no significant differences between the two groups in gen-eral patient characteristics.The experimental group had significantly shorter operation time,duration of drainage tube placement,and length of hospital stay compared to the control group.Intraoperative blood loss,VAS scores,and C-reactive protein(CRP)levels were significantly lower in the experimental group(P<0.05 for all).There were no significant differences in preoperative and postoperative blood creatinine,plasma osmolality(Posm),and white blood cell count(WBC)between the two groups(P>0.05).The incidence of postoperative complications was similar be-tween the two groups(P>0.05).Conclusion:Percutaneous nephrolithotomy combined with thulium laser cystecto-my for simple renal cysts is a minimally invasive,time-efficient,low-bleeding,and fast-recovery treatment modality,demonstrating safety and efficacy.
