经尿道前列腺铥激光剜除术治疗前列腺增生合并A型血友病患者1例
A Case of Transurethral Thulium Laser Enucleation for Benign Prostatic Hyperplasia in a Patient with Hemophilia A

摘要
回顾性分析1例经尿道前列腺铥激光剜除术治疗前列腺增生合并A型血友病患者临床资料.围术期进行外源性凝血因子Ⅷ制剂替代治疗,手术过程顺利,出血少,术后第 2日停膀胱冲洗,术后第 3天拔除尿管,患者排尿通畅,尿色微红.围术期未输血,无明显出血、血栓等不良事件.提示围术期进行外源性FⅧ制剂替代治疗下,经尿道前列腺铥激光剜除术治疗前列腺增生合并血友病A是一种相对安全有效的手术方式,具有一定的临床参考意义.
Abstract
A retrospective analysis of the clinical data of a patient with benign prostatic hyperplasia(BPH)com-plicated by hemophilia A who underwent transurethral thulium laser enucleation of the prostate.During the periopera-tive period,exogenous coagulation factor Ⅷ(FⅧ)replacement therapy was administered.The surgery was per-formed smoothly with minimal blood loss.Bladder irrigation was discontinued on the second postoperative day,and the urinary catheter was removed on the third postoperative day.The patient experienced smooth urination,with slightly reddish urine.No blood transfusion was required during the perioperative period,and there were no significant adverse events such as bleeding or thrombosis.This case suggests that under perioperative exogenous FⅧ replace-ment therapy,transurethral thulium laser enucleation of the prostate in patients with BPH complicated by hemophilia A is a relatively safe and effective surgical approach,with certain clinical reference value.
