产后出血并发弥散性血管内凝血1例报道

摘要
产后出血是产科一种严重的并发症,是导致产妇死亡的主要原因之一.产后出血是指自然分娩后 24 h内阴道出血量>500 ml,剖宫产术后 24 h内阴道出血>1000 ml,严重时还会继发弥散性血管内凝血(DIC),造成产妇溶血性贫血、休克,甚至死亡.本文报道平邑县人民医院收治的1例因停经 40 周+,少量阴道流血>3 h就诊,患者分娩后出现难治性产后出血,并发DIC、腹腔弥漫渗血,病情危重转入重症医学科进一步诊治.现回顾性分析该病例的特点及诊治情况,为临床诊治产后出血并发DIC提供依据.
Abstract
Postpartum hemorrhage is a serious complication in obstetrics and one of the main causes of maternal death.Postpartum hemorrhage is defined as vaginal bleeding>500 ml within 24 hours after natural delivery and>1000 ml after cesarean section.In severe cases,it will be secondary to disseminated intravascular coagulation(DIC),resulting in maternal hemolytic anemia,shock,and even death.This paper reported a case of 40+ weeks of amenorrhea,a small amount of vaginal bleeding for more than 3 hours.The patient developed refractory postpartum hemorrhage after delivery,complicated by DIC and diffuse oozing blood in the abdominal cavity.The patient was transferred to the intensive care unit for further diagnosis and treatment.The characteristics and diagnosis and treatment of this case were retrospectively analyzed to provide the basis for clinical diagnosis and treatment of postpartum hemorrhage complicated with DIC.
