无保护会阴接生联合自由体位分娩在自然分娩产妇中的应用

摘要
目的 探讨无保护会阴接生联合自由体位分娩在自然分娩产妇中的应用效果.方法 选取2022 年1月至 2023 年 5 月在贵州省人民医院产科进行分娩的138例产妇为研究对象,按照随机数字表法分为对照组(传统体位分娩)和研究组(无保护会阴接生联合自由体位分娩),每组各 69 例,观察两组分娩情况.结果 研究组会阴侧切率低于对照组,Ⅰ度会阴裂伤发生率高于对照组,Ⅱ度会阴裂伤发生率低于对照组(P<0.05);研究组第一产程、第二产程及总产程短于对照组(P<0.05);研究组产后出血发生率、产后疼痛评分低于对照组,产后 2 h出血量少于对照组(P<0.05).结论 无保护会阴接生法联合自由体位分娩能够缓解分娩疼痛,缩短产程,降低会阴裂伤程度、侧切率及产后出血发生率.
Abstract
Objective To explore the application effect of unprotected perineum delivery combined with free position delivery in natural delivery puerpera.Methods A total of 138 pregnant women who delivered in the Department of Obstetrics of Guizhou Provincial People's Hospital from January 2022 to May 2023 were selected as the research objects.According to the random number table method,they were divided into the control group(traditional position delivery)and the study group(unprotected perineum delivery combined with free position delivery),with 69 cases in each group.Childbirth of two groups was observed.Results The rate of episiotomy in the study group was lower than that in the control group,the incidence of Ⅰ degree perineal laceration was higher than that in the control group,and the incidence of Ⅱ degree perineal laceration was lower than that in the control group(P<0.05).The first stage of labor,the second stage of labor and the total stage of labor in the study group were shorter than those in the control group(P<0.05).The incidence of postpartum hemorrhage and postpartum pain score in the study group were lower than those in the control group,and the amount of bleeding 2 hours after delivery was less than that in the control group(P<0.05).Conclusion Unprotected perineum delivery combined with free position delivery can relieve labor pain,shorten stage of labor,reduce the degree of perineal laceration,the rate of lateral incision and the incidence of postpartum hemorrhage.
