480例不孕症女性X线子宫输卵管造影检查结果分析

摘要
目的 分析480 例不孕症女性X线子宫输卵管造影检查结果.方法 选取 2022 年 2 月至12 月于山东省威海市妇幼保健院生殖医学科就诊的 480 例不孕症患者为研究对象,均接受X线子宫输卵管造影检查,分析X线子宫输卵管造影检查结果及不同类型子宫、输卵管异常的X线特征.结果 480 例中,子宫、输卵管均正常占 30.00%,输卵管异常占 51.25%,子宫异常占18.75%.输卵管炎症X线表现为不规则和粗细不均匀的管壁,输卵管可见粘连或迂曲,活动度较小;输卵管畸形X线表现为输卵管发育不良,伴有狭窄或阻塞等情况;输卵管阻塞X线表现为壶腹部形状为囊袋状扩大或壶腹部与峡部呈腊肠状扩大,输卵管边缘光滑,造影剂不能顺利进入盆腔内;不完全阻塞表现为壶腹部与峡部均伴有轻度扩张,仅有较少的造影剂流入盆腔,且速度缓慢,可见部分显影;子宫异常表现为子宫内部毛糙不整齐,内壁不光滑,内膜凹凸不平,同时可见大小不等的充盈缺损,宫腔中伴有粘连,角部膨粗且变形,子宫畸形可见子宫大小或者位置等异常.结论 X线子宫输卵管造影检查在女性不孕症检查中应用,能够明确子宫、输卵管的异常情况,为临床诊治提供依据.
Abstract
Objective To analyze the results of X-ray hysterosalpingography in 480 infertile women.Methods A total of 480 infertility patients who were treated in the Department of Reproductive Medicine,Weihai Maternal and Child Health Hospital of Shandong Province from February to December 2022 were selected as the research objects.All of them underwent X-ray hysterosalpingography.The results of X-ray hysterosalpingography and the X-ray characteristics of different types of uterine and fallopian tube abnormalities were analyzed.Results Among 480 cases,30.00%had normal uterus and fallopian tube,51.25%had abnormal fallopian tube,and 18.75%had abnormal uterus.The X-ray of fallopian tube inflammation showed irregular and uneven tube wall,adhesion or tortuosity of fallopian tube,and small mobility.Fallopian tube malformation X-ray showed fallopian tube dysplasia,accompanied by stenosis or obstruction.Fallopian tube obstruction X-ray showed that the ampulla was enlarged in the shape of a pouch or the ampulla and isthmus were enlarged in the shape of a sausage,the edge of the fallopian tube was smooth,and the contrast agent could not enter the pelvic cavity smoothly.Incomplete obstruction was manifested as mild expansion of ampulla and isthmus,only a small amount of contrast agent flowed into the pelvic cavity,and the speed was slow,and part of the development could be seen.The uterine abnormalities were rough and irregular inside the uterus,the inner wall was not smooth,the endometrium was uneven,filling defects of different sizes could be seen at the same time,adhesion was observed in the uterine cavity,the corner was expanded and deformed,and uterine malformation could be abnormal in the size or position of the uterus.Conclusion The application of X-ray hysterosalpingography in the examination of infertile women can clarify the abnormalities of the uterus and fallopian tubes,and provide a basis for clinical diagnosis and treatment.
