子宫内膜癌保育治疗后复发的影响因素分析

摘要
目的 分析子宫内膜癌(EC)保育治疗后复发的影响因素.方法 选取 2019 年1月至 2021年1月于湖北医药学院附属十堰市太和医院妇科进行治疗的102 例EC患者作为研究对象,根据患者复发情况分为复发组(n=30)和未复发组(n=72),采用单因素及多因素logistic回归分析影响EC保育治疗后复发的因素.结果 两组体重指数(BMI)≥24 kg/m2、病灶大小≥2 cm比较,差异有统计学意义(P<0.05);两组POLE超突变型、CN-H型比较,差异有统计学意义(P<0.05);多因素logistic回归分析结果显示,BMI≥24 kg/m2、病灶大小≥2 cm、CN-H型是EC保育治疗后复发的危险因素(P<0.05).结论 BMI≥24 kg/m2、病灶大小≥2 cm和CN-H型是EC患者保育治疗后复发的影响因素,临床应根据实际情况,及时调整治疗方案,保障患者预后.
Abstract
Objective To analyze the influencing factors of recurrence after fertility-sparing treatment in endometrial cancer(EC).Methods A total of 102 EC patients who were treated in the Department of Gynecology,Shiyan Taihe Hospital Affiliated to Hubei University of Medicine from January 2019 to January 2021 were selected as the research objects.According to the recurrence of EC,the patients were divided into recurrence group(n=30)and non-recurrence group(n=72).Univariate and multivariate logistic regression analysis were used to analyze the factors for recurrence after fertility-sparing treatment in EC.Results There were significant differences in body mass index(BMI)≥24 kg/m2 and lesion size≥2 cm between the two groups(P<0.05).There were significant differences in POLE hypermutation and CN-H type between the two groups(P<0.05).Multivariate logistic regression analysis showed that BMI≥24 kg/m2,lesion size≥2 cm,and CN-H type were risk factors for recurrence of fertility-sparing treatment in EC(P<0.05).Conclusion BMI≥24 kg/m2,lesion size≥2 cm and CN-H type are the influencing factors of recurrence in EC patients after fertility-sparing treatment.The clinic should adjust the treatment program in time according to the actual situation to guarantee the patients'prognosis.
