无创高频振荡通气与无创正压通气治疗新生儿呼吸窘迫综合征的临床效果比较

摘要
目的 比较无创高频振荡通气(NHFOV)与无创正压通气(NPPV)治疗新生儿呼吸窘迫综合征(NRDS)的临床效果.方法 选取 2020 年 5 月至 2023 年 5 月临沂市妇幼保健院收治的120 例NRDS患儿,按照随机数字表法分为对照组和观察组,每组各 60 例.对照组予以NPPV治疗,观察组予以NHFOV治疗.比较两组血气指标、肺功能指标、炎症因子水平及并发症发生情况.结果 治疗后,观察组各血气指标均优于对照组(P<0.05).治疗后,观察组潮气量、达峰容积比及达峰时间比均高于对照组(P<0.05).治疗后,观察组白介素-6、肿瘤坏死因子-α及白介素-8 水平均低于对照组(P<0.05).观察组并发症总发生率低于对照组(P<0.05).结论 NHFOV治疗NRDS的临床效果较好,可有效改善血气指标及炎症因子水平,加速肺功能恢复,减少并发症的发生.
Abstract
Objective To compare the clinical effect of noninvasive high-frequency oscillatory ventilation(NHFOV)and noninvasive positive pressure ventilation(NPPV)in the treatment of neonatal respiratory distress syndrome(NRDS).Methods A total of 120 children with NRDS treated in Linyi Maternal and Child Health Hospital from May 2020 to May 2023 were selected and divided into the control group and the observation group according to the random number table method,with 60 cases in each group.The control group was treated with NPPV and the observation group was treated with NHFOV.Blood gas indexes,lung function indexes,inflammatory factor levels and complications were compared between the two groups.Results After treatment,the blood gas indexes in the observation group were better than those in the control group(P<0.05).After treatment,tidal volume,ratio of volume at peak tidal expiratory flow to expiratory tidal volume and ratio of time to peak tidal expiratory flow to total expiratory time in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of interleukin-6,tumor necrosis factor-α and interleukin-8 in the observation group were lower than those in the control group(P<0.05).The total complication rate of the observation group was lower than that of the control group(P<0.05).Conclusion NHFOV has a good clinical effect in the treatment of NRDS,which can effectively improve the blood gas indexes and inflammatory factors levels,accelerate the recovery of lung function,and reduce the occurrence of complications.
