摘要
目的 分析儿童社区获得性肺炎(CAP)多重耐药菌(MDRO)感染的危险因素.方法 选取 2019 年2 月至 2023 年 2 月山东大学附属威海市立医院收治的 62 例CAP患儿,采用单因素及多因素logistic回归分析CAP患儿MDRO感染的危险因素.结果 62 例CAP患儿中,发生MDRO感染 25 例(40.32%).单因素分析结果显示,CAP患儿MDRO感染与年龄<3 岁、早产、胸部X线片提示两肺受累、营养不良、既往抗菌药使用时间≥7 d有关(P<0.05).多因素logistic回归分析显示,早产、营养不良、既往抗菌药使用时间≥7 d是CAP患儿MDRO感染的独立危险因素(P<0.05).结论 早产、营养不良、既往抗菌药使用时间≥7 d是CAP患儿MDRO感染的独立危险因素,临床需准确识别MDRO感染高风险患儿,尽早采取针对性干预措施.
Abstract
Objective To analyze the risk factors of multidrug-resistant organism(MDRO)infection in children with community-acquired pneumonia(CAP).Methods A total of 62 children with CAP admitted to Weihai Municipal Hospital Affiliated to Shandong University from February 2019 to February 2023 were selected to analyze the risk factors of MDRO infection in children with CAP by univariate and multivariate logistic regression.Results Among 62 children with CAP,25(40.32%)had MDRO infection.The results of univariate analysis showed that MDRO infection in children with CAP was associated with age<3 years old,preterm birth,lung involvement indicated by chest X-ray,malnutrition,and previous antibiotic use≥7 days(P<0.05).Multivariate logistic regression analysis showed that preterm birth,malnutrition,and previous antibiotic use≥7 days were independent risk factors for MDRO infection in CAP children(P<0.05).Conclusion Preterm birth,malnutrition,and previous antibiotic use≥7 days are independent risk factors for MDRO infection in CAP children.It is necessary to accurately identify children with high risk of MDRO infection and take targeted intervention measures as soon as possible.
