经脐单孔腹腔镜在子宫肌瘤剔除术中的应用效果及对切口美观度的影响

摘要
目的 探讨经脐单孔腹腔镜在子宫肌瘤剔除术中的应用效果及对切口美观度的影响.方法 选取2020 年1月至 2023 年10 月广东同江医院收治的 60 例子宫肌瘤患者作为研究对象,按照随机数字表法分为对照组和观察组,每组各 30 例.对照组采用多孔腹腔镜子宫肌瘤剔除术治疗,观察组采用经脐单孔腹腔镜子宫肌瘤剔除术治疗.比较两组围手术期指标、术后疼痛程度、并发症发生情况及切口美观度.结果 两组住院时间比较,差异无统计学意义(P>0.05).观察组手术时间、肛门排气时间均短于对照组,术中出血量少于对照组(P<0.05).两组并发症总发生率比较,差异无统计学意义(P>0.05).观察组术后 6、24、48、72 h视觉模拟评分法评分均低于对照组(P<0.05).观察组切口美容量表评分高于对照组(P<0.05).结论 经脐单孔腹腔镜在子宫肌瘤剔除术中的应用效果较好,患者疼痛感轻,并发症少,切口美观度高,治疗效果显著.
Abstract
Objective To explore the application effect of transumbilical single-port laparoscopy in hystero-myomectomy and its influence on aesthetic degree of incision.Methods A total of 60 patients with uterine fibroids admitted to Guangdong Tongjiang Hospital from January 2020 to October 2023 were selected as the study objects.All the patients were divided into the control group and the observation group according to the random number table method,with 30 cases in each group.The control group was treated with porous laparoscopic hysteromyomectomy,and the observation group was treated with transumbilical single-port laparoscopy in hysteromyomectomy.The perioperative indicators,the levels of postoperative pain,the incidence of complications and aesthetic degree of incision were compared between the two groups.Results There was no significant difference in the hospitalization time between the two groups(P>0.05).The operation time and anal exhaust time of the observation group were shorter than those of the control group,the intraoperative bleeding volume was less than that of the control group(P<0.05).There was no statistically significant difference in the total incidence of complications between the two groups(P>0.05).The scores of visual analogue scale at 6,24,48 and 72 hours after surgery in the observation group were lower than those in the control group(P<0.05).The score of circumferential scar in the observation group was higher than that in the control group(P<0.05).Conclusion The application effect of transumbilical single-port laparoscopy in hysteromyomectomy is better,with mild pain,fewer complications,high incision aesthetics and significant treatment effect.
