摘要
目的 探讨保乳术联合乳腔镜腋窝淋巴结清扫术治疗早期乳腺癌的效果.方法 选取2021年 9 月至 2022 年12 月淄博市妇幼保健院乳腺甲状腺外科收治的 66 例早期乳腺癌患者作为研究对象,按照单双号分组法分为对照组和观察组,每组各 33 例.对照组行保乳术联合常规腋窝淋巴结清扫术,观察组行保乳术联合乳腔镜腋窝淋巴结清扫术.比较两组肩关节活动度、并发症总发生率、引流管留置时间及术后引流量.结果 术后 3 个月,两组肩关节活动度均小于术前,但观察组大于对照组(P<0.05).观察组并发症总发生率低于对照组(P<0.05).观察组引流管留置时间短于对照组,术后引流量少于对照组(P<0.05).结论 保乳术联合乳腔镜腋窝淋巴结清扫术治疗早期乳腺癌,可减少对肩关节活动度的影响,降低并发症发生率,缩短引流管留置时间,减少术后引流量,值得临床推广.
Abstract
Objective To investigate the effect of breast conserving surgery combined with endoscopic axillary lymph node dissection in the treatment of early breast cancer.Methods A total of 66 patients with early breast cancer admitted to the Department of Breast and Thyroid Surgery of Zibo Maternal and Child Health Hospital from September 2021 to December 2022 were selected as the study objects,and were divided into the control group and the observation group according to odd-even grouping method,with 33 cases in each group.The control group underwent breast conserving surgery combined with conventional axillary lymph node dissection,and the observation group underwent breast conserving surgery combined with endoscopic axillary lymph node dissection.The range of motion of shoulder joint,the total incidence of complications,the retention time of drainage tube and the postoperative drainage volume were compared between the two groups.Results Three months after surgery,the range of motion of shoulder joint in both groups was smaller than that before surgery,but that in the observation group was larger than that in the control group(P<0.05).The total complication rate of the observation group was lower than that of the control group(P<0.05).The retention time of drainage tube in the observation group was shorter than that in the control group,and the postoperative drainage volume in the observation group was less than that in the control group(P<0.05).Conclusion Breast conserving surgery combined with endoscopic axillary lymph node dissection in the treatment of early breast cancer can reduce the influence on the range of motion of shoulder joint,reduce the incidence of complications,shorten the retention time of drainage tube and reduce the postoperative drainage volume,which is worthy of clinical promotion.
