摘要
目的 探讨右美托咪定联合罗哌卡因硬膜外麻醉在剖宫产中的应用效果.方法 选取在北京市昌平区中西医结合医院 2021年1月至 2022 年12 月行硬膜外麻醉剖宫产的150 例产妇,按照随机数字表法将其分为两组,每组 75 例.对照组给予罗哌卡因,观察组给予罗哌卡因联用右美托咪定,比较两组麻醉情况及皮质醇(Cor)、心率(HR)、去甲肾上腺素(NA)、平均动脉压(MAP)水平.结果 观察组麻醉达峰、运动最大阻滞、麻醉起效和恢复运动时间短于对照组,维持麻醉平面时间长于对照组(P<0.05);两组麻醉后15 min(T1)、胎儿娩出后(T2)、手术结束后(T3)的Cor、HR、NA和MAP水平与麻醉前比较,差异有统计学意义(P<0.05);与对照组比较,观察组T1、T2、T3 的Cor、HR、NA水平更低,MAP水平更高(P<0.05).结论 右美托咪定联合罗哌卡因硬膜外麻醉在剖宫产中应用,能改善麻醉情况及麻醉相关临床指标.
Abstract
Objective To investigate the application effect of Dexmedetomidine combined with Ropivacaine for epidural anesthesia in cesarean section.Methods A total of 150 parturients who underwent cesarean section under epidural anesthesia in Beijing Changping District Hospital of Traditional Chinese and Western Medicine from January 2021 to December 2022 were selected and divided into two groups according to the random number table method,with 75 cases in each group.The control group was given Ropivacaine,and the observation group was given Ropivacaine combined with Dexmedetomidine.The anesthesia,the levels of cortisol(Cor),heart rate(HR),norepinephrine(NA),and mean arterial pressure(MAP)were compared between the two groups.Results The time of peak anesthesia,maximum movement block,onset of anesthesia and recovery of movement in the observation group was shorter than that in the control group,and the time of maintaining anesthesia plane was longer than that in the control group(P<0.05).The levels of Cor,HR,NA,and MAP at 15 min after anesthesia(T1),after fetal delivery(T2),and after operation(T3)in the two groups were compared with those before anesthesia,and the differences were statistically significant(P<0.05).Compared with the control group,the levels of Cor,HR,and NA at T1,T2,and T3 in the observation group were lower,and the level of MAP was higher(P<0.05).Conclusion Dexmedetomidine combined with Ropivacaine for epidural anesthesia in cesarean section can improve anesthesia and anesthesia related clinical indicators.
