液基薄层细胞检测联合高危型人乳头瘤病毒检测在宫颈癌前病变诊断中的价值

摘要
目的 分析液基薄层细胞检测(TCT)联合高危型人乳头瘤病毒(HPV)检测在宫颈癌前病变诊断中的价值.方法 选取 2023 年1月至 6 月兰州市妇幼保健院收治的487 例疑似宫颈癌前病变患者作为研究对象,均接受TCT、高危型HPV检查,以病理检查作为金标准,分析不同检测方法的诊断效能.结果 487 例患者中,病理检查共检出131例宫颈癌前病变.以病理检查结果作为金标准,TCT联合高危型HPV检查的灵敏度、准确度、阴性预测值高于单一检查方法(P<0.05).Kappa检验显示,TCT联合高危型HPV检查结果与病理检查结果具有极好的一致性(Kappa=0.969,P=0.000).结论 TCT联合高危型HPV检测诊断宫颈癌前病变具有较高的灵敏度与准确度,值得临床推广应用.
Abstract
Objective To analyze the value of thin-prep cytology test(TCT)combined with high-risk human papilloma virus(HPV)detection in the diagnosis of cervical precancerous lesions.Methods A total of 487 patients with suspected cervical precancerous lesions admitted to Lanzhou Maternal and Child Health Hospital from January to June 2023 were selected as the study objects.All of them received TCT and high-risk HPV examination,and pathological examination was used as the gold standard to analyze the diagnostic efficacy of different detection methods.Results Among the 487 patients,131 cases of precancerous lesions were detected by pathological examination.Using the results of pathological examination as the gold standard,the sensitivity,accuracy and negative predictive value of TCT combined with high-risk HPV detection were higher than those of single examination(P<0.05).Kappa test showed that the results of TCT combined with high-risk HPV detection had excellent consistency with the results of pathological examination(Kappa=0.969,P=0.000).Conclusion TCT combined with high-risk HPV detection in the diagnosis of cervical precancerous lesions has high sensitivity and accuracy,and is worthy of clinical application.
