摘要
目的 分析妊娠期急性脂肪肝(AFLP)的临床特征.方法 选取 2010 年1月至 2020 年 9 月滨州市人民医院收治的 22 例AFLP患者,分析AFLP的临床特征.结果 22 例患者中,16 例发生消化道症状,且均有不同程度的黄疸,14例纤维蛋白原<1.5 g/L,7 例白蛋白<20 g/L,3 例发生肝性脑病,6 例并发肾衰竭,5 例并发多器官功能障碍综合征,2例胎死宫内.结论 对AFLP患者及时终止妊娠,积极预防弥散性血管内凝血,补充凝血物质,根据病情及时给予输血及血制品输注、血液置换及血液净化等对症治疗,有利于改善AFLP患者及胎儿的预后.
Abstract
Objective To analyze the clinical characteristics of acute fatty liver of pregnancy(AFLP).Methods A total of 22 patients with AFLP admitted to Binzhou People's Hospital from January 2010 to September 2020 were selected,and the clinical characteristics of AFLP were analyzed.Results Among the 22 patients,16 cases had gastrointestinal symptoms,and all had different degrees of jaundice,14 cases had fibrinogen<1.5 g/L,7 cases had albumin<20 g/L,3 cases had hepatic encephalopathy,6 cases had renal failure,5 cases had multiple organ dysfunction syndrome,and 2 cases had intrauterine death.Conclusion Timely termination of pregnancy for AFLP patients,active prevention of disseminated intravascular coagulation,supplementation of clotting substances,timely transfusion and transfusion of blood products,blood replacement,and blood purification and other symptomatic treatment according to the condition are conducive to improving the prognosis of AFLP patients and fetuses.
