摘要
目的 分析经会阴盆底超声在产后膀胱膨出诊断中的应用价值.方法 回顾性分析 2022 年 3 月至 2023 年 5 月在淄博市妇幼保健院超声科进行经会阴盆腔超声检查的 87 例产妇,其中 30 例产后膀胱膨出产妇为膀胱膨出组,57例无产后膀胱膨出的产妇为参照组.比较两组膀胱颈移动度与肛提肌裂孔面积,并分析诊断价值.结果 膀胱膨出组膀胱颈移动度、肛提肌裂孔面积均大于参照组,差异有统计学意义(P<0.001).膀胱颈移动度诊断的曲线下面积(AUC)为 0.809,其灵敏度、特异度分别为 80.00%、84.20%.肛提肌裂孔面积诊断的AUC为 0.811,其灵敏度、特异度分别为 76.70%、87.70%.结论 经会阴盆腔超声检查可有效、直观地观察盆腔脏器变化,可为产后膀胱膨出提供诊断依据,并辅助明确膨出程度,具有临床应用价值.
Abstract
Objective To analyze the application value of transperineal pelvic floor ultrasound in the diagnosis of postpartum cystocele.Methods A retrospective analysis was performed on 87 parturients who underwent transperineal pelvic floor ultrasound examination in the Department of Ultrasound of Zibo Maternal and Child Health Hospital from March 2022 to May 2023.Thirty parturients with postpartum cystocele were in the cystocele group,and 57 parturients without postpartum cystocele were in the reference group.The bladder neck mobility and hiatus area of levator anal muscle were compared between the two groups,and the diagnostic value was analyzed.Results The bladder neck mobility and hiatus area of levator anal muscle in the cystocele group were larger than those in the reference group,and the differences were statistically significant(P<0.001).The area under the curve(AUC)of bladder neck mobility diagnosis was 0.809,and its sensitivity and specificity were 80.00%and 84.20%,respectively.The AUC of hiatus area of levator anal muscle diagnosis was 0.811,and its sensitivity and specificity were 76.70%and 87.70%,respectively.Conclusion Transperineal pelvic floor ultrasound can effectively and intuitively observe the changes of pelvic organs,provide diagnostic basis for postpartum cystocele,and assist in determining the degree of expansion,which has clinical application value.
