X线消化道造影在小儿先天性巨结肠中的诊断价值

摘要
目的 探讨X线消化道造影在小儿先天性巨结肠中的诊断价值.方法 选取 2021年11月至2023 年11月在新疆维吾尔自治区儿童医院诊治的 72 例疑似小儿先天性巨结肠患儿作为研究对象,均进行X线消化道造影检查,将直肠黏膜活检与病理活检作为金标准,评估X线消化道造影在小儿先天性巨结肠中的诊断效能.结果 72 例患儿中,活检诊断为先天性巨结肠49 例,占比 68.06%,其中超短段 30 例(41.67%),短段4例(5.56%),长段 3 例(4.17%),常见型 8 例(11.11%),全结肠4例(5.56%).72 例患儿中,X线消化道造影诊断先天性巨结肠 50 例(69.44%),其中超短段 29 例(40.28%),短段 4 例(5.56%),长段 5 例(6.94%),常见型 8 例(11.11%),全结肠4例(5.56%).X线消化道造影诊断小儿先天性巨结肠的敏感性、特异性、准确性分别为100.00%(49/49)、95.65%(22/23)、98.61%(71/72).结论 X线消化道造影诊断小儿先天性巨结肠具有较高的敏感性、特异性、准确性,值得临床推广应用.
Abstract
Objective To investigate the diagnostic value of X-ray gastrointestinal angiography in children with Hirschsprung's disease.Methods A total of 72 children with suspected Hirschsprung's disease diagnosed and treated in Children's Hospital of Xinjiang Uygur Autonomous Region from November 2021 to November 2023 were selected as the study subjects.All of them underwent X-ray gastrointestinal angiography.Rectal mucosal biopsy and pathological biopsy were taken as the gold standard to evaluate the diagnostic efficacy of X-ray gastrointestinal angiography in children with Hirschsprung's disease.Results Among the 72 children,49 cases(68.06%)were diagnosed as Hirschsprung's disease by biopsy,including 30 cases(41.67%)of ultra-short segment,4 cases(5.56%)of short segment,3 cases(4.17%)of long segment,8 cases(11.11%)of common type,and 4 cases(5.56%)of whole colon.Among the 72 children,50 cases(69.44%)were diagnosed with Hirschsprung's disease by X-ray gastrointestinal angiography,including 29 cases(40.28%)of ultra-short segment,4 cases(5.56%)of short segment,5 cases(6.94%)of long segment,8 cases(11.11%)of common type,and 4 cases(5.56%)of whole colon.The sensitivity,specificity,and accuracy of X-ray gastrointestinal angiography in the diagnosis of Hirschsprung's disease in children were 100.00%(49/49),95.65%(22/23),and 98.61%(71/72),respectively.Conclusion The diagnosis of children with Hirschsprung's disease by X-ray gastrointestinal angiography has high sensitivity,specificity,and accuracy,and is worthy of clinical application.
