摘要
目的 探讨彩色多普勒超声在乳腺良恶性肿瘤鉴别诊断中的价值.方法 选取 2022 年1月至2023 年 6 月在单县妇幼保健院就诊的166 例疑似乳腺肿瘤患者,均进行彩色多普勒超声检查,以病理检查结果作为金标准,分析诊断效能,比较不同病理性质超声征象、血流参数.结果 病理检查确诊乳腺良性肿瘤100 例,乳腺恶性肿瘤 66 例.彩色多普勒超声检查检出乳腺良性肿瘤 99 例,乳腺恶性肿瘤 67 例,敏感度为96.97%,特异度为 97.00%,准确性为 96.99%,阳性与阴性预测值分别为 95.52%、97.98%.乳腺恶性肿瘤中形态不规则、边界模糊、无包膜、微钙化、内部中高回声、周围组织浸润、血流信号丰富的占比及阻力指数、搏动指数、收缩期峰值流速最大值、收缩期峰值平均流速均高于乳腺良性肿瘤(P<0.05).结论 彩色多普勒超声在乳腺良恶性肿瘤鉴别诊断中的价值较高,临床可推广.
Abstract
Objective To investigate the value of color Doppler ultrasound in differential diagnosis of benign and malignant breast tumors.Methods From January 2022 to June 2023,166 patients with suspected breast tumors who were treated in Shan County Maternal and Child Health Hospital were selected to undergo color Doppler ultrasound examination,and pathological examination results were used as the gold standard to analyze diagnostic efficiency,and compare ultrasonic signs and blood flow parameters of different pathological properties.Results Pathological examination confirmed 100 cases of benign breast tumors and 66 cases of malignant breast tumors.The sensitivity,specificity,and accuracy of color Doppler ultrasound were 96.97%,97.00%and 96.99%,respectively.The positive and negative predictive values were 95.52%and 97.98%,respectively.The proportion of irregular shape,blurred boundary,no envelope,microcalcification,medium and high internal echo,surrounding tissue infiltration,rich blood flow signal,resistance index,pulsation index,maximum peak systolic flow velocity,and average peak systolic flow velocity in malignant breast tumors were higher than those in benign breast tumors(P<0.05).Conclusion Color Doppler ultrasound is of high value in the differential diagnosis of benign and malignant breast tumors and can be popularized in clinic.
