0~3岁儿童血常规指标与25羟维生素D关系的横断面研究

摘要
目的 分析0~3岁儿童血常规指标与25羟维生素D[25(OH)D]的关系.方法 选取2018年1月至 2022 年12 月在北京万柳美中宜和妇儿医院保健体检的851名 0~3 岁儿童纳入横断面研究.采用单因素相关及回归分析、多因素线性回归模型分析平均红细胞体积(MCV)与 25(OH)D的关系.结果 851名0~3 岁儿童中,25(OH)D水平为16.75~212.95 nmol/L,平均(84.54±31.11)nmol/L,维生素D缺乏14例(1.65%),维生素D不足108例(12.69%),无维生素D过量;血红蛋白水平为83~151 g/L,轻度贫血66例(7.76%),中度贫血 3 例(0.35%),无重度贫血,总贫血率为8.11%;红细胞计数为(3.25~6.23)×1012/L,MCV为 52.8~98.8 fl.25(OH)D低水平组与高水平组MCV、母亲生育年龄、体检月份比较,差异有统计学意义(P<0.05).单因素相关分析:Pearson相关系数(r)=0.105,P=0.001.单因素回归分析:方程检验F=9.401,P=0.002;回归系数β=0.737(95%CI 0.265~1.209),t=3.066,P=0.002.多因素线性回归分析显示,MCV与 25(OH)D呈正相关;每年 5 月1日至10 月 31日体检的儿童 25(OH)D水平较高;月龄增大,25(OH)D呈下降趋势(P<0.05).结论 0~3 岁儿童MCV与 25(OH)D水平呈正相关,可利用MCV作为 25(OH)D变化的敏感指标,在血常规检查中发现MCV水平较低者可考虑关注其维生素D营养状况.
Abstract
Objective To analyze the relationship between blood routine indexes and 25-hydroxyvitamin D[25(OH)D]in children aged 0 to 3 years.Methods A total of 851 children aged 0 to 3 years who underwent physical examination in Beijing Wanliu Amcare Women's and Children's Hospital from January 2018 to December 2022 were selected for the cross-sectional study.The relationship between mean corpuscular volume(MCV)and 25(OH)D was analyzed by single factor correlation and regression analysis and multifactor linear regression model.Results Among the 851 children aged 0 to 3 years,the 25(OH)D levels ranged from 16.75 to 212.95 nmol/L,with an average of(84.54±31.11)nmol/L.There were 14 cases(1.65%)of vitamin D deficiency,108 cases(12.69%)of vitamin D inadequate,and no excessive vitamin D.The hemoglobin level was 83-151 g/L,66 cases were mild anemia(7.76%),3 cases were moderate anemia(0.35%),no severe anemia,the total anemia rate was 8.11%.Red blood cell count was(3.25-6.23)×1012/L,MCV was 52.8-98.8 fl.There were significant differences in MCV,maternal childbearing age,and physical examination month between 25(OH)D low level group and 25(OH)D high level group(P<0.05).Univariate correlation analysis:Pearson correlation coefficient(r)=0.105,P=0.001.Univariate regression analysis:Equation test F=9.401,P=0.002;Regression coefficient β=0.737(95%CI 0.265-1.209),t=3.066,P=0.002.Multifactor linear regression analysis showed that MCV was positively correlated with 25(OH)D.Higher levels of 25(OH)D were found in children who were examined between 1 May and 31 October each year,and levels of 25(OH)D decreased with the increase of monthly age(P<0.05).Conclusion MCV is positively correlated with the 25(OH)D level in children aged 0 to 3 years.MCV can be used as a sensitive index for the change of 25(OH)D.If the MCV level is found to be low in the blood routine examination,the nutritional status of vitamin D should be concerned.
