宫颈液基细胞学检查联合阴道镜Reid评分在宫颈癌前病变筛查中的应用价值

摘要
目的 分析宫颈液基细胞学检查联合阴道镜Reid评分在宫颈癌前病变筛查中的应用价值.方法 选取 2023 年1月至10 月在兰州市妇幼保健院门诊筛查宫颈癌前病变的 277例患者作为研究对象,进行宫颈液基细胞学检查及阴道镜检查,以宫颈组织病理学诊断为金标准,分析宫颈液基细胞学检查联合阴道镜Reid评分在宫颈癌前病变筛查中的应用价值.结果 宫颈组织病理学诊断结果显示,277例筛查宫颈癌前病变患者中,阳性119 例,阴性158例,联合筛查结果与宫颈组织病理学诊断结果的Kappa值为 0.896,高于宫颈液基细胞学检查与宫颈组织病理学诊断结果的Kappa值(0.722),且高于阴道镜Reid评分与宫颈组织病理学诊断结果的Kappa值(0.755).宫颈液基细胞学检查、阴道镜Reid评分联合筛查宫颈癌前病变的灵敏度、阳性预测值高于宫颈液基细胞学检查,准确度高于二者单独筛查,差异有统计学意义(P<0.017).结论 宫颈液基细胞学检查、阴道镜Reid评分在宫颈癌前病变筛查中均具有一定的应用价值,两者联合可提升筛查价值.
Abstract
Objective To analyze the application value of cervical liquid based cytology combined with colposcopy Reid score in the screening of cervical precancerous lesions.Methods A total of 277 patients with cervical precancerous lesions were screened in the Outpatient Department of Lanzhou Maternal and Child Health Hospital from January to October 2023.Cervical liquid based cytology and colposcopy were performed.Taking cervical histopathological diagnosis as the gold standard,the application value of cervical liquid based cytology combined with colposcopy Reid score in cervical precancerous lesions screening was analyzed.Results The cervical histopathological diagnosis results showed that among 277 patients with cervical precancerous lesions,119 cases were positive and 158 cases were negative.The Kappa value of the combined screening results and cervical histopathological diagnosis results was 0.896,which was higher than that of the cervical liquid based cytology and cervical histopathological diagnosis(Kappa=0.722),and higher than that of the colposcopy Reid score and cervical histopathological diagnosis(Kappa=0.755).The sensitivity and positive predictive value of the combination of cervical liquid based cytology and colposcopy Reid score in screening cervical precancerous lesions were higher than those of cervical liquid based cytology,the accuracy was higher than that of the two screening methods alone,and the differences were statistically significant(P<0.017).Conclusion Cervical liquid based cytology and colposcopy Reid score have certain application value in the screening of cervical precancerous lesions,and the combination of the two examination can improve the screening value.
