基于加速康复外科理念的围手术期营养管理在先天性巨结肠患儿中的应用

摘要
目的 探讨基于加速康复外科理念的围手术期营养管理在先天性巨结肠患儿中的应用效果.方法 选取 2020 年 8 月至 2023 年4月右江民族医学院附属医院小儿外科收治的 66 例先天性巨结肠患儿为研究对象,其中 2020 年 8 月至 2021年12 月入院的患儿纳入对照组(33 例),2022 年1月至 2023 年 4 月入院的患儿纳入干预组(33 例).对照组给予常规护理,干预组给予基于加速康复外科理念的围手术期营养管理.比较两组的营养学指标、生活质量.结果 干预组出院 3 个月后的儿科营养不良筛查表评分低于对照组,术后第 7 天、出院 3 个月后的总蛋白及血红蛋白水平高于对照组(P<0.05);干预组出院 3 个月后的儿童生活质量普适性核心量表评分高于对照组(P<0.05).结论 基于加速康复外科理念的围手术期营养管理能够改善先天性巨结肠患儿的营养状态,提高生活质量,值得推广应用.
Abstract
Objective To explore the application effect of perioperative nutrition management based on the concept of enhanced recovery after surgery in children with Hirschsprung disease.Methods A total of 66 children with Hirschsprung disease in Department of Pediatrics Surgery,Affiliated Hospital of Youjiang Medical University for Nationalities from August 2020 to April 2023 were selected as the study objects.Among them,those admitted from August 2020 to December 2021 were included in the control group(33 cases),and those admitted from January 2022 to April 2023 were included in the intervention group(33 cases).The control group was given routine nursing,and the intervention group was given perioperative nutrition management based on the concept of enhanced recovery after surgery.The nutritional indexes and quality of life in the two groups were compared.Results The score of screening tool for the assessment of malnutrition in pediatrics at 3 months after discharge in the intervention group was lower than that in the control group,and the levels of total protein and hemoglobin at 7 days after surgery and 3 months after discharge were higher than those in the control group(P<0.05).The pediatric quality of life inventory version 4.0 generic core scale score at 3 months after discharge in the intervention group was higher than that in the control group(P<0.05).Conclusion Perioperative nutrition management based on the concept of enhanced recovery after surgery can improve the nutritional status and quality of life in children with Hirschsprung disease,which is worth popularizing and applying.
